大黄[庶虫]虫片联合胃复春胶囊治疗慢性萎缩性胃炎伴肠上皮化生的临床观察

摘　　要：目的探析大黄[庶虫]虫片联合胃复春胶囊治疗慢性萎缩性胃炎(chronic atrophic gastritis,CAG)伴肠上皮化生(intestinal metaplasia,IM)的临床疗效。方法选取2022年3月至2023年1月浙江中医药大学附属湖州市中医院收治的50例脾虚络瘀型CAG伴IM患者为研究对象,根据随机数字表法将其分为观察组和对照组,每组各25例。对照组患者予口服瑞巴派特片治疗,观察组患者予大黄[庶虫]虫片联合胃复春胶囊治疗,两组患者均连续用药3个月。比较两组患者的临床疗效、治疗前后中医证候积分、D-二聚体、胃蛋白酶原Ⅰ(pepsinogenⅠ,PG-Ⅰ)、IM积分。结果观察组患者的总有效率显著高于对照组(χ^(2)=11.590,P=0.009);治疗后,两组患者的中医证候积分、D-二聚体水平均显著低于本组治疗前,血清PG-Ⅰ水平均显著高于本组治疗前(P<0.05),观察组患者的中医证候积分显著低于对照组(P<0.05);观察组患者治疗前后的D-二聚体、PG-Ⅰ差值均显著大于对照组(P<0.05)。观察组患者治疗前后的IM积分差值显著优于对照组(P<0.05)。结论大黄[庶虫]虫片联合胃复春胶囊治疗CAG伴IM的临床疗效显著,可有效减轻患者的临床症状,提高血清PG-Ⅰ水平,减轻胃黏膜萎缩与IM程度,具有临床推广使用价值。Objective To explore the clinical effect of Dahuang Zhechong tablet combined with Weifuchun capsule in the treatment of chronic atrophic gastritis(CAG)with intestinal metaplasia(IM).Methods Fifty patients with spleen deficiency and stagnant blockade of collateral type CAG with IM admitted to Huzhou Hospital of Traditional Chinese Medicine of Zhejiang Chinese Medical University from March 2022 to January 2023 were selected as the study objects.They were divided into observation group and control group according to random number table method,with 25 cases in each group.The control group was treated with oral administration of rebamipide tablet,while the observation group was treated with a combination of Dahuang Zhechong tablet and Weifuchun capsule.Both groups were treated continuously for three months.The clinical efficacy,TCM syndrome scores,D-dimer,pepsinogenⅠ(PG-Ⅰ),and IM score before and after treatment were compared between two groups.Results The total effective rate of observation group was significantly higher than that of control group(χ^(2)=11.590,P=0.009).After treatment,the TCM syndrome score and D-dimer level of patients in two groups were significantly lower than before treatment,and the serum PG-Ⅰlevel was significantly higher than before treatment(P<0.05).The TCM syndrome score in observation group were significantly lower than that in control group(P<0.05).The difference values of D-dimer and PG-Ⅰin observation group before and after treatment were significantly higher than those in control group(P<0.05).The difference of IM score before and after treatment in observation group was significantly better than that in control group(P<0.05).Conclusion Dahuang Zhechong tablet combined with Weifuchun capsule has significant clinical efficacy in treatment of CAG with IM,which can effectively relieve the clinical symptoms of patients,improve the serum PG-Ⅰlevel,reduce the degree of gastric mucosa atrophy and IM,and has clinical promotion value.
