血清同型半胱氨酸水平与卒中后癫痫发作的相关性分析

摘  要:目的探讨血清同型半胱氨酸(Hcy)水平与卒中后癫痫发作的相关性。方法选取2021年1—12月菏泽市立医院收治的47例卒中后癫痫患者作为观察组,选取同期收治的47例卒中未合并癫痫患者作为对照组,另选取同期于本院体检的50名健康体检者作为健康组。所有研究对象进行血清Hcy水平检测,比较3组血清Hcy水平;采用单因素及多因素Logistic回归分析卒中后癫痫发作的相关影响因素;绘制ROC曲线分析Hcy检测对卒中后癫痫发作的预测价值;采用Pearson相关系数分析Hcy水平与卒中后癫痫发作的相关性。结果观察组血清Hcy水平[(24.25±2.53)μmol/L]高于对照组[(17.26±2.02)μmol/L]和健康组[(7.42±1.13)μmol/L],且对照组高于健康组,差异有统计学意义(P<0.05)。单因素分析显示,观察组病灶部位为皮质比例、血清Hcy水平均高于对照组,差异有统计学意义(P<0.05);多因素Lo-gistic回归分析结构显示,血清Hcy水平、病灶部位在皮质为卒中后癫痫发作的高危因素(OR>1,P<0.05)。ROC曲线分析结果显示,血清Hcy水平检测预测卒中后癫痫发作的AUC为0.858,预测价值良好。Pearson相关分析结果显示,血清Hcy水平与卒中后癫痫发作呈正相关性(r=0.617,P<0.05)。结论血清Hcy水平、病灶部位在皮质为卒中后癫痫发作的高危因素,血清Hcy水平与卒中后癫痫发作呈正相关,检测其水平变化有助于临床预测卒中后癫痫发作情况,并及时开展针对性干预。Objective To explore the correlation between serum homocysteine(Hcy)levels and post-stroke epilepsy.Method 47 post-stroke epilepsy patients admitted to Heze Municipal Hospital from January to December 2021 were selected as the observation group,47 stroke patients without concurrent epilepsy admitted during the same period were selected as the control group,and 50 healthy individuals who underwent physical examinations in our hospital during the same period were selected as the healthy group.Serum Hcy levels were detected in all subjects,and the serum Hcy levels were compared among the three groups;Univariate and multivariate logistic regression were used to analyze the related influencing factors of post-stroke epilepsy;ROC curve was drawn to analyze the predictive value of Hcy detection in post-stroke epileptic;Pearson correlation coefficient was used to analyze the correlation between Hcy levels and post-stroke.Results Hcy level in the observation group([24.25±2.53]μmol/L)was higher than that in the control group([17.26±2.02]μmol/L)and the healthy group([7.42±1.13]μmol/L),and the control group was higher than the healthy group,the differences were statistically significant(P<0.05).Univariate analysis showed that the proportion of lesion location in the cortex and serum Hcy levels in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05);multivariat Logistic e analysis showed that serum Hcy level and lesion location in the cortex were high-risk factors for post-stroke epilepsy(OR>1,P<0.05).ROC curve analysis showed that the AUC of serum Hcy level in predicting post-stroke epileptic was 0.858,which had a good predictive value.Pearson correlation analysis showed that serum Hcy levels was positively correlated with post-stroke epileptic(r=0.617,P<0.05).Conclusion serum Hcy level and lesion location in the cortex were high-risk factors for post-stroke epilepsy,the serum Hcy level is positively correlated with post-stroke epilepsy,and detecti
