牙隐裂伴慢性牙髓炎患牙根管治疗术后5年临床疗效评价及影响因素分析

摘　　要：目的通过回顾性分析牙隐裂伴慢性牙髓炎患牙根管治疗术后5年的存留率及影响因素分析,为隐裂牙的临床治疗提供参考。方法选取本院2015年1月至2016年12月收治的71例牙隐裂伴慢性牙髓炎的患牙作为研究对象,行完善的根管治疗。术后5年回访,回顾性分析患者的年龄、性别、牙位、牙周情况、隐裂纹的分布及深度和是否行全冠修复,同时对影响因素进行相关性分析。结果术后5年回访,2例患者失访,69例患者纳入研究,牙隐裂伴慢性牙髓炎患牙五年存留率为85.51%。患牙牙周情况和是否进行全冠修复是患牙五年存留率的独立影响因素(P<0.05)。结论通过对隐裂纹的深度,患牙牙周情况可评估隐裂牙的预后,完善根管治疗后的隐裂牙建议全冠修复以提高患牙的存留率。Objective To evaluate the 5-year survival rate and influencing factors of root canal therapy in patients with cracked teeth complicated with chronic pulpitis by retrospective analysis,and to provide for the clinical treatment.Methods A total of 71 teeth diagnosed as cracked teeth with chronic pulpitis range from January 2015 to December 2016 were inclued.Root canal therapy effects were evaluated after 5-year follow-up.Age,gender,tooth position,distribution of the cracks,depth of the cracks,periodontal status and full crown restoration of the patients were analyzed,and correlation analysis method were used to identify the main factors of cracked teeth with chronic pulpitis.Results 69 patients were evaluated after 5-year follow-up and 2 patients were lost.The overall survival rate was 85.51%.Multivariate analysis showed that periodontal status and full crown restoration were independent influencing factors for the effective survival rate(P<0.05).Conclusion The therapeutic effects of cracked teeth with chronic pulpitis can be efficiently predicted by the depth of cracks and the periodontal status.Postoperative full crown restoration could increase the survival rate of cracked teeth with chronic pulptitis.
