复方丹参滴丸联合丁苯酞软胶囊对血管性痴呆的治疗效果研究

摘　　要：目的 探究复方丹参滴丸联合丁苯酞软胶囊治疗血管性痴呆(VaD)的临床效果。方法 选取2020年1月—2022年12月收治的150例VaD,根据随机数字表法分组,观察组50例予复方丹参滴丸联合丁苯酞软胶囊治疗,对照A组50例予复方丹参滴丸治疗,对照B组50例予丁苯酞软胶囊治疗,2周为1个疗程,治疗2个疗程后比较治疗效果;并比较3组治疗前后简易精神状态量表(MMSE)、临床痴呆评定量表(CDR)评分、Barthel(BI)指数,治疗前后内质网应激途径相关蛋白(CHOP、GRP78、GRP94)表达、血清炎性因子[白细胞介素-1β(IL-1β)、IL-6、IL-8]、氧化应激指标[丙二醛(MDA)、超氧化物歧化酶(SOD)、脂质过氧化物(LPO)]水平,以及不良反应发生情况。结果 观察组临床总有效率[92.00%(46/50)]高于对照A、B组[74.00%(37/50)、76.00%(38/50)](P<0.05);治疗1个、2个疗程后,观察组MMSE评分、BI指数高于对照A、B组,CDR评分低于对照A、B组(P<0.05);治疗1个疗程、2个疗程后,观察组GRP78、GRP94及CHOP蛋白表达和血清IL-1β、MDA、IL-6、LPO、IL-8水平低于对照A、B组,SOD水平高于对照A、B组(P<0.05)。3组均未出现明显不良反应。结论 复方丹参滴丸联合丁苯酞软胶囊治疗VaD较两药单独治疗效果好,可改善患者认知功能,减轻痴呆症状,提升生活自理能力,还可抑制内质网应激、减轻机体炎症及氧化应激反应,且安全性高。Objective To explore the therapeutic effect of compound Danshen dropping pills combined with Butadiphthalein softgel on vascular dementia(VaD).Methods A total of 150 patients with VaD from January 2020 to December 2022 were selected and divided into groups according to random number table method.Fifty patients in observation group were treated with compound Danshen dropping pills combined with Butylphthalide softgel,50 patients in control group A were treated with compound Danshen dropping pills,and 50 patients in control group B were treated with Butylphthalide softgel for 1 course of 2 weeks.The therapeutic effect was compared after 2 courses of treatment.The clinical effects,the score of Mini-mental State Examination(MMSE),Clinical Dementia Rating Scale(CDR)scores and Barthel(BI)of the three groups were compared before and after treatment.Expression of endoplasmic reticulum stress pathway-related proteins(CHOP,GRP78,GRP94),serum inflammatory factors[interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-8(IL-8)],oxidative stress indexes[malondialdehyde(MDA),superoxide dismutase(SOD),lipid peroxidase(LPO)]before and after treatment,and the occurrence of adverse reactions were compared before and after treatment.Results The total effective rate of observation group[92.00%(46/50)]was higher than that of control groups A and B[74.00%(37/50),76.00%(38/50)](P<0.05).After 1 and 2 courses of treatment,MMSE score and BI index in observation group were higher than those in control groups A and B,while CDR score was lower than that in control groups A and B(P<0.05).After 1 course and 2 courses of treatment,the expression of GRP78,GRP94 and CHOP protein and serum levels of IL-1β,MDA,IL-6,LPO and IL-8 in observation group were lower than those in control groups A and B,while the SOD level in observation group was higher than that in control groups A and B(P<0.05).There were no obvious adverse reactions in the three groups.Conclusion Compared with the two drugs alone,compound Danshen dropping pills combined with Butylopht
