摘  要:目的 探究髋骨初级凸轮形态与骨骼成熟度及髋关节周围解剖结构的相关性。方法 回顾性分析2016—2021年于我院急诊科就诊行腹盆腔CT扫描青少年。其中凸轮形态组30例(58侧髋),健康对照组30例(60侧髋)。分别评估2组股骨近端骨骺成熟状态,测量Alpha角(AA)、股骨颈干角(NSA)、骨骺伸展(EE)、中心-边缘角(CEA)。统计学方法采用χ^(2)检验,t检验,Pearson相关性分析。结果 凸轮形态组股骨近端骨骺成熟度在性别、年龄及AA差异均有统计学意义(P<0.05),凸轮形态好发于男性,且骨骺开放状态较骨骺闭合状态时具有更大的AA。凸轮形态组AA、EE均较对照组增大,凸轮形态组NSA较对照组减小,差异均有统计学意义(P<0.05);且AA与NSA成负相关,与EE成正相关;2组间CEA差异无统计学意义,且CEA和AA相关性较低。结论 初级凸轮形态好发于男性,且在股骨近端骨骺闭合前发展。初级凸轮形态、髋关节内翻及骨骺线延伸是股骨近端骨骺闭合前对作用于髋关节应力的一种适应性表现,且存在相关性。Objective To explore the correlation between primary cam morphology and skeletal maturity and anatomy around the hip joint.Methods A retrospective analysis was performed on adolescents who underwent abdominal and pelvic CT scans in the emergency department of our hospital from 2016 to 2021.The mature state of the proximal femoral epiphysis was assessed,and the Alpha angle(AA),neck-shaft angle(NSA),epiphyseal extension(EE),and center edge angle(CEA)were measured.χ^(2) test,t test and Pearson correlation analysis were used in this study.Results There were significant differences in gender,age and AA in the maturity of proximal femoral epiphysis in the cam shape group(P<0.05).The cam shape was more common in males,and the open epiphysis had greater AA than the closed epiphysis.The differences in AA,NSA,and EE between the two groups were statistically significant.The AA and EE in the cam morphology group were increased compared with the control group,and the NSA in the cam morphology group was decreased compared with the control group.There was a negative correlation between AA and NSA,and a positive correlation between AA and EE.The difference in CEA between the two groups was not statistically significant,and the correlation between CEA and AA was low.Conclusion The primary cam morphology is more common in males and develops before the closure of the proximal femoral epiphysis.The primary cam shape,hip varus and epiphyseal line extension are adaptive manifestations of the stress acting on the hip joint before the proximal femoral epiphysis is closed,and there are correlations.
