摘　　要：目的探究胃癌患者中HER2/c-erbB-2蛋白表达特征,为治疗和预后的指导价值作进一步研究。方法我院于2020年7月至2021年7月收治的100例胃癌患者被选为受试对象,治疗方案均为手术治疗,术中选择病灶及周围组织(距离病灶直径超于3 cm),采用免疫组化法对组织表达能力作检验;根据患者病理特点及随访资料,比较阳性表达率与胃癌病理特点、生存期之间的关系。采用Cox回归模型进行预后生存影响因素分析,采用Kaplan-Merier进行生存分析。结果淋巴结转移患者的HER2/c-erbB-2阳性率高于未转移者,TNM分期的Ⅲ-Ⅳ期HER2/c-erbB-2阳性率高于Ⅰ-Ⅱ期(P<0.05);病灶组织HER2/c-erbB-2阳性率高于癌旁组织(P<0.05);淋巴结转移、TNM分期和HER2/c-erbB-2为影响胃癌患者预后生存的独立危险因素(P<0.05)。HER2/c-erbB-2蛋白表达阳性患者生存期短于阴性患者(P<0.05)。结论HER2/c-erbB-2蛋白在胃部肿瘤表达程度高,具有缩短生存期的作用,对胃癌的靶向治疗有指导意义。Objective To explore the expression characteristics of HER2/c-erbB-2 protein in patients with gastric cancer,and to further study the guiding value of treatment and prognosis.Methods 100 patients with gastric cancer admitted to our hospital from July 2020 to July 2021 were selected as subjects.The treatment options were all surgical treatment.The lesion and surrounding tissues were selected during the operation(the diameter of the lesion was more than 3 cm).Immunohistochemical method was used to test the tissue expression ability;according to the pathological characteristics of patients and follow-up data,the relationship between the positive expression rate and the pathological characteristics and survival period of gastric cancer was compared.The Cox regression model was used to analyze the prognostic survival factors,and the Kaplan-Merier was used to analyze the survival.Results The positive rate of HER2/c-erbB-2 in patients with lymph node metastasis was higher than that in those without metastasis,and the positive rate of HER2/c-erbB-2 in TNM stage III-IV stage was higher than that of stage I-II(P<0.05);The positive rate of HER2/c-erbB-2 in tissues was higher than that in adjacent tissues(P<0.05);Lymph node metastasis,TNM staging and HER2/c-erbB-2 are independent risk factors affecting the prognosis and survival of patients with gastric cancer(P<0.05);the survival time of patients with positive HER2/c-erbB-2 protein expression was shorter than that of negative patients(P<0.05).Conclusion The HER2/c-erbB-2 protein is highly expressed in gastric tumors and has the effect of shortening the survival period,which has guiding significance for the targeted therapy of gastric cancer.
