摘　　要：目的探讨小脑扁桃体下疝畸形Ⅰ型(CM-Ⅰ)手术预后的相关性因素。方法回顾性分析新疆医科大学第一附属医院神经外科2016-08—2023-01手术治疗的80例CM-Ⅰ患者的临床资料,行芝加哥预后评分(CCOS),分析性别、年龄、高血压、糖尿病、BMI、病程、术前脊髓空洞、术前神经功能缺损、扁桃体凸出程度、术式等因素对患者手术预后的影响,确定CM-Ⅰ手术预后的相关因素。结果年龄(OR=0.015,95%CI:0.001~0.448,P=0.015)、病程(OR=0.020,95%CI:0.001~0.477,P=0.016)、术前脊髓空洞(OR=0.017,95%CI:0.000~0.916,P=0.045)及术式(OR=0.042,95%CI:0.002~0.984,P=0.049)是CM-Ⅰ手术预后的独立影响因素。结论CM-Ⅰ手术预后的相关性因素包括年龄、病程、术前脊髓空洞及术式。Objective To investigate the prognostic factors for Chiari malformation typeⅠ(CM-Ⅰ).Methods Clinical data of 80 CM-Ⅰpatients treated in the Neurosurgery Department of the First Affiliated Hospital of Xinjiang Medical University from August,2016 to January,2023 were retrospectively analyzed.Chicago prognostic score(CCOS)was performed on the patients.The gender,age,hypertension,diabetes,BMI,course of disease,preoperative syringomyelia,preoperative nerve defect,tonsil protration degree,surgical style and other factors on the surgical prognosis of patients were analyzed,and the relevant factors for the prognosis of CM-Ⅰsurgery were determined.Results Among the many factors considered,age(OR=0.015,95%CI:0.001-0.448,P=0.015),course of disease(OR=0.020,95%CI:0.001-0.477,P=0.016),preoperative syringomyelia(OR=0.017,95%CI:0.000-0.916,P=0.045)and surgical style(OR=0.042,95%CI:0.002-0.984,P=0.049)had statistical differences.Conclusion Prognostic factors for CM-Ⅰsurgery include age,course of disease,preoperative syringomyelia,and surgical method.
