CK20、CDX2、CK7组织表达水平对结直肠癌术后疗效及预后的预测价值

摘　　要：目的探讨CK20、CDX2、CK7组织表达水平对结直肠癌术后疗效及预后的预测价值。方法收集60例结直肠癌患者的基本资料进行回顾性分析。患者均行直肠癌根治术,采用免疫组化SP法对肿瘤组织中CK20、CDX2、CK7表达进行检测,并对患者进行为期1年的随访,通过复查结肠镜,根据患者术后有无肿瘤复发或转移将其分为预后良好组(n=26)和预后不良组(n=34),多因素Logistic回归分析影响直肠癌患者预后因素。结果与术前相比较,术后CK20、CK7在结直肠癌组织中的阳性表达率(80.00%、83.33%)有所降低,CDX2的阳性表达率(76.66%)则有所提高(P<0.05);多因素Logistic分析显示,包块大小(P=0.034,OR=1.411)、CK20(P=0.024,OR=1.850)、CDX2(P=0.013,OR=1.540)、CK7(P=0.012,OR=1.464)表达水平是结直肠癌患者预后的独立影响因素。结论CK20、CDX2、CK7组织表达水平在结直肠癌的发生、发展中发挥重要作用,且在对结直肠癌术后疗效及预后评估方面具有一定的临床应用价值。Objective To explore the predictive value of CK20,CDX2,CK7 tissue expression levels for postoperative curative effect and prognosis of colorectal cancer.Methods The basic data of 60 patients with colorectal cancer were collected and analyzed retrospectively.All patients underwent radical resection of rectal cancer.Immunohistochemical SP method was used to detect the expression of CK20,CDX2 and CK7 in tumor tissues,and the patients were followed up for one year.By reexamining colonoscopy,the patients were divided into a good prognosis group(n=26)and a poor prognosis group(n=34)according to whether the tumor recurred or metastasized after operation.Univariate analysis and multivariate Logistic regression analysis were used to analyze the influencing factors of the prognosis of colorectal cancer patients.Results Compared with before operation,after operation,the positive expression rates of CK20 and CK7 in colorectal cancer(80.00%,83.33%)decreased,but the positive expression rate of CDX2(76.66%)increased(P<0.05).The Logistic regression model showed that the mass size(P=0.034,OR=1.411),CK20(P=0.024,OR=1.850),CDX2(P=0.013,OR=1.540),CK7(P=0.012,OR=1.540).Conclusion The expression levels of CK20,CDX2 and CK7 play an important role in the occurrence and development of colorectal cancer,and have certain clinical application value in evaluating the postoperative curative effect and prognosis of colorectal cancer.
