摘 要:目的:分析伽玛刀立体定向放射治疗肺癌脑转移瘤患者预后不良的影响因素。方法:回顾性分析2022年1月至2023年1月在该院接受伽马刀立体定向放射治疗的121例肺癌脑转移瘤患者的临床资料。统计伽玛刀立体定向放射治疗肺癌脑转移瘤患者的预后情况,采用Logistic回归分析伽玛刀立体定向放射治疗肺癌脑转移瘤患者预后不良的影响因素。结果:121例伽马刀立体定向放射治疗肺癌脑转移瘤患者预后不良40例,设为预后不良组,预后良好81例,设为预后良好组;预后不良组脑转移瘤最大直径≥3 cm、血清鳞状细胞癌抗原(SCC-Ag)≥3.80 ng/mL、血清细胞角蛋白19片段(CYFRA21-1)≥7.52 ng/mL、血清神经元特异性烯醇化酶(NSE)≥50.81μg/L占比均高于预后良好组,差异有统计学意义(P<0.05);Logistic回归分析结果显示,血清SCC-Ag≥3.80 ng/mL、血清NSE≥50.81μg/L均为伽玛刀立体定向放射治疗肺癌脑转移瘤患者预后不良的危险因素(OR>1,P<0.05)。结论:血清SCC-Ag≥3.80 ng/mL、血清NSE≥50.81μg/L均为伽玛刀立体定向放射治疗肺癌脑转移瘤患者预后不良的危险因素。Objective:To analyze influencing factors of poor prognosis in patients with brain metastases of lung cancer treated by Gamma knife stereotactic radiotherapy.Methods:The clinical data of 121 patients with brain metastases of lung cancer treated with Gamma knife stereotactic radiotherapy in this hospital from January 2022 to January 2023 were retrospectively analyzed.The prognosis of these patients was statistically analyzed.Logistic regression was used to analyze the influencing factors of poor prognosis in the patients with brain metastases of lung cancer treated with Gamma knife stereotactic radiotherapy.Results:Among the 121 cases with brain metastases of lung cancer treated by Gamma knife stereotactic radiotherapy,40 cases had poor prognosis and were set as the poor prognosis group,and 81 cases had good prognosis and were set as the good prognosis group.The proportions of the patients with maximum diameter of brain metastases≥3 cm,serum SCC-Ag≥3.80 ng/mL,serum CYFRA21-1≥7.52 ng/mL,and serum NSE≥50.81μg/L in the poor prognosis group were higher than those of the good prognosis group,and the differences were statistically significant(P<0.05).The Logistic regression analysis showed that serum SCC-Ag≥3.80 ng/mL and serum NSE≥50.81μg/L were risk factors for poor prognosis in the patients with brain metastases of lung cancer treated with Gamma knife stereotactic radiotherapy(OR>1,P<0.05).Conclusions:Serum SCC-Ag≥3.80 ng/mL and serum NSE≥50.81μg/L are the risk factors for poor prognosis in the patients with brain metastases of lung cancer treated with Gamma knife stereotactic radiotherapy。
