内镜下套扎术与泡沫硬化剂注射术治疗Ⅱ、Ⅲ度内痔的临床研究

摘　　要：目的探讨内镜下套扎术和内镜下泡沫硬化剂注射术治疗Ⅱ、Ⅲ度内痔的疗效及安全性。方法回顾性分析2020年1月—2022年12月本院收治的行内镜下套扎术和内镜下泡沫硬化剂注射术治疗的Ⅱ、Ⅲ度内痔患者共24例的临床资料,其中套扎组12例,硬化组12例。比较两组患者的一般资料(性别、年龄、内痔分度等),统计两组患者的围手术期情况(套扎环数、注射剂量、注射点数、住院时间),术后24 h应用视觉模拟评分(visual analogue scale,VAS)评估疼痛程度,术后1周分析并发症发生率,术后3个月统计有效性。结果术后24 h套扎组有91.67%的患者出现疼痛,以重度疼痛为主(50.00%),而硬化组仅有41.67%的患者出现轻度疼痛(P<0.05)。术后1周硬化组的并发症发生率(8.33%)低于套扎组(75.00%)(P<0.05)。随访3个月,硬化组的总有效率(91.67%)较套扎组(66.67%)高(P<0.05)。两组患者的住院时间无差异性(P>0.05)。结论与内镜下套扎术比较,内镜下泡沫硬化剂注射术治疗Ⅱ、Ⅲ度内痔的有效率高且并发症发生率低,值得临床推广。Objective To investigate the efficacy and safety of endoscopic variceal ligation and endoscopic foam sclerotherapy in the treatment of gradeⅡorⅢinternal hemorrhoids.Methods The clinical data of 24 patients with gradeⅡorⅢinternal hemorrhoids who were treated with endoscopic variceal ligation or endoscopic foam sclerotherapy in the first affiliated hospital of Bengbu Medical College from January 2020 to December 2022 was retrospectively analyzed.There were 12 cases in the ligation group and 12 cases in the sclerosis group.The general data of patients in the two groups were compared(including gender,age,grade of internal hemorrhoids etc).Perioperative indexes(including number of ligation loop,dose of injection,number of injection site,hospitalization time)were statistically analyzed.The pain level of the two groups was evaluated by score of visual analogue scale(VAS)at 24 h after treatment.The incidence rate of complications at 1 week after operation was analyzed.The efficacy of treatment was assessed after follow-up of 3 months.Results 91.67%of patients in the ligation group experienced pain within 24 hours after operation,most of them were severe pain(50.00%).Only 41.67%of patients in the sclerosis group felt mild pain(P<0.05).The incidence rate of complications at 1 week after operation in the sclerosis group(8.33%)was lower than that of ligation group(75.00%)(P<0.05).After 3 months of follow-up,the total effective rate of sclerosis group(91.67%)was higher than that(66.67%)of ligation group(P<0.05).There was no significant difference in hospitalization time(P>0.05).Conclusions Compared with endoscopic variceal ligation,endoscopic foam sclerotherapy could achieve higher total effective rate and lower complications incidence rate in the treatment of gradeⅡorⅢinternal hemorrhoids,which is worthy of clinical application widely.
