摘　　要：目的比较子宫肌瘤患者采用宫腔镜子宫肌瘤电切术(HH)与腹腔镜子宫肌瘤剔除术(LM)的效果。方法选择子宫肌瘤患者100例为研究对象,根据手术类型不同,分为LM组和HH组,每组50例。LM组行LM术,HH组行HH术。比较两组患者的围术期指标、术后并发症发生情况、肌瘤复发率及妊娠率。结果HH组手术时长、胃肠功能恢复时长、体温恢复正常时长、住院时长均较LM组短,总出血量较LM组低,差异有统计学意义(P<0.05)。HH组并发症发生率为6.00%,低于LM组的24.00%,差异有统计学意义(P<0.05)。术后3、6、8个月及术后1年,HH组妊娠率均较LM组高,差异有统计学意义(P<0.05)。两组肌瘤复发率比较,差异无统计学意义(P>0.05)。结论HH术与LM术均能有效治疗子宫肌瘤,相较而言,HH术具有手术时长短、对机体创伤小、术后并发症发生率低、妊娠率高等优点。Objective To compare the effect of hysteroscopic hysteromyomectomy(HH)and laparoscopic myomectomy(LM)in patients with uterine fibroids.Methods 100 patients with uterine fibroids were selected as the study objects and were divided into a LM group and a HH group according to different types of surgery,with 50 cases in each group.LM was performed in LM group and HH was performed in HH group.The perioperative indexes,postoperative complications,fibroid recurrence rate and pregnancy rate were compared between the two groups.Results The operation time,gastrointestinal function recovery time,normal temperature recovery time and hospital stay time in the HH group were shorter than those in the LM group,and the total blood loss was lower than that of the LM group,the differences were statistically significant(P<0.05).The incidence of complications in the HH group was 6.00%,lower than 24.00%in the LM group,and the difference was statistically significant(P<0.05).At 3,6,8 months and 1 year after operation,the pregnancy rates in the HH group were higher than those in the LM group,and the differences were statistically significant(P<0.05).There was no significant difference in the recurrence rate of fibroids between the two groups(P>0.05).Conclusion HH and LM are both effective in the treatment of uterine fibroids.HH has the advantages of shorter operation time,less trauma to the body,lower incidence of postoperative complications and higher pregnancy rate.
