TCT联合HPV-DNA检测在宫颈病变筛查中的应用

摘　　要：目的探讨液基薄层细胞学检测(TCT)联合人乳头瘤病毒遗传物质(HPV-DNA)检测在宫颈病变筛查中的应用价值。方法回顾性分析167例接受宫颈病变筛查女性的临床资料,均进行TCT、HPV-DNA单独与联合检测及阴道镜下宫颈组织活检,以阴道镜下宫颈组织活检为“金标准”,分析TCT、HPV-DNA单独与联合检测的诊断效能。结果TCT检测的敏感性为85.71%(12/14),特异性为91.50%(140/153),准确性为91.02%(152/167),阳性预测值为48.00%(12/25),阴性预测值为98.59%(140/142)。HPV-DNA检测的敏感性为92.86%(13/14),特异性为92.16%(141/153),准确性为92.22%(154/167),阳性预测值为52.00%(13/25),阴性预测值为99.30%(141/142)。联合检测的敏感性为100%(14/14),特异性为83.01%(127/153),准确性为84.43%(141/167),阳性预测值为35.00%(14/40),阴性预测值为100%(127/127)。TCT、HPV-DNA联合检测的敏感性高于单独检测,HPV-DNA检测的灵敏度高于TCT检测(P<0.05)。结论宫颈病变筛查中,TCT、HPV-DNA联合检测的应用价值较单独检测高。Objective To explore the application value of thinprep cytologic test(TCT)and human papillomavirus genetic material(HPV-DNA)combined detection in cervical lesion screening.Methods The clinical data of 167 women who underwent cervical lesion screening were retrospectively analyzed.TCT,HPV-DNA alone and combined detection and colposcopic cervical tissue biopsy were performed.Using colposcopic cervical biopsy as the"gold standard",the diagnostic efficacy of TCT and HPVDNA alone and combined detection was analyzed.Results The sensitivity of TCT detection was 85.71%(12/14),specificity was 91.50%(140/153),accuracy was 91.02%(152/167),positive predictive value was 48.00%(12/25),and negative predictive value was 98.59%(140/142).The sensitivity of HPV-DNA detection was 92.86%(13/14),specificity was 92.16%(141/153),accuracy was 92.22%(154/167),positive predictive value was 52.00%(13/25),and negative predictive value was 99.30%(141/142).The sensitivity of the combined detection was 100%(14/14),specificity was 83.01%(127/153),accuracy was 84.43%(141/167),positive predictive value was 35.00%(14/40),and negative predictive value was 100%(127/127).The sensitivity of TCT and HPV-DNA combined detection was higher than those of individual detection,while the sensitivity of HPV-DNA detection was higher than that of TCT detection(P<0.05).Conclusion In the screening of cervical lesions,the combined detection of TCT and HPV-DNA is more valuable than the singledetection.
