摘 要:目的分析Ⅰ型子宫内膜癌患者中缺氧诱导因子-1(HIF-1)、E-钙黏蛋白(E-cad)的表达及其相关性。方法选取子宫内膜非典型增生病变患者、子宫内膜健康者、Ⅰ型子宫内膜癌患者各30例,所有受试者均接受HIF-1、E-cad检测,观察其检测结果。结果Ⅰ型子宫内膜癌患者HIF-1阳性率83.33%(25/30)高于子宫内膜非典型增生病变患者36.67%(11/30)子宫内膜健康者10.00%(3/30),E-cad阳性率33.33%(10/30)低于子宫内膜非典型增生病变患者93.33%(28/30)、子宫内膜健康者96.67%(29/30),差异有统计学意义(P<0.05)。分析得知,Ⅰ型子宫内膜癌患者E-cad阳性表达与病理学分期、肌层浸润程度、淋巴结转移有关(P<0.05),HIF-1阳性表达均与组织学分级、病理学分期、肌层浸润程度、淋巴结转移有关(P<0.05)。Spearman分析显示,Ⅰ型子宫内膜癌中E-cad与HIF-1成负相关(r=-0.561,P<0.05)。结论E-cad、HIF-1均在子宫内膜癌病变中有参与性,联合检测E-cad、HIF-1在判定Ⅰ型子宫内膜癌患者和病变侵袭性方面具有临床价值。Objective The expression and correlation of hypoxia-inducible factor-1(HIF-1)and calmodulin E(E-cad)in patients with endometrial carcinoma typeⅠwere analyzed.Methods 30 patients with endometrial atypical hyperplasia,healthy endometrial and typeⅠendometrial cancer were selected.All subjects were tested by HIF-1 and E-cad,and the detection results were observed.Results The positive rate of HIF-1 in patients with typeⅠendometrial cancer was 83.33%(25/30)higher than that in patients with atypical hyperplasia of endometrial disease 36.67%(11/30)and healthy endometrial patients 10.00%(3/30).The positive rate of E-cad was 33.33%(10/30)lower than that of patients with atypical endometrial hyperplasia 93.33%(28/30)and healthy endometrial 96.67%(29/30),and the difference was statistically significant(P<0.05).The positive expression of E-cad in patients with typeⅠendometrial carcinoma was correlated with pathological stage,degree of myoinfiltration and lymph node metastasis(P<0.05),and positive expression of HIF-1 was correlated with histological grade,pathological stage,degree of myoinfiltration and lymph node metastasis(P<o.05).Spearman analysis showed that E-cad was negatively correlated with HIF-1 in typeⅠendometrial carcinoma(r=-0.561,P<0.05).Conclusion E-cad(up-regulated)and HIF-1(down-regulated)are involved in endometrial cancer.Combined detection of E-cad and HIF-1 has clinical value in determining patients with typeⅠendometrial cancer and the aggressiveness of the lesions.
