摘　　要：目的本研究旨在分析双相情感障碍抑郁发作患者自杀和生物学指标的关联,为评估和监测提供依据。方法本研究采用回顾性研究方法,对首都医科大学附属北京安定医院2014年1月至2017年12月期间住院的双相情感障碍抑郁发作患者的病历资料进行横断面研究。通过电子病历系统,收集患者的人口学、临床和生物学指标数据,并使用R(version 4.2.2)统计学软件进行分析。结果共纳入2027例双相情感障碍抑郁发作患者,其中417例(20.6%)伴有自杀意念或行为。与不伴自杀的患者相比,自杀组患者女性和躯体合并症的比例更高,血清IgA和睾酮水平更高(均P<0.05)。多元Logistic回归分析显示,IgA和躯体合并症与伴自杀意念和行为显著正向关联。结论高血清IgA水平以及伴有躯体合并症极可能是双相情感障碍抑郁发作患者发生自杀的危险因素,建议作为早期风险识别的指标。Objective To analyze the association between suicidality and biological parameters in patients with bipolar depression episodes and to provide a basis for evaluation and monitoring.Methods A retrospective cross-sectional study was conducted on hospitalized patients with bipolar depression episodes at Beijing Anding Hospital,Capital Medical University,from January 2014 to December 2017.The patient data including demographic,clinical,and biological parameters were collected from the electronic medical records system and analyzed using R(version 4.2.2)statistical software.Results A total of 2027 patients with bipolar depression episodes were included,with 417(20.6%)exhibiting suicidal ideation or behavior.Compared to the non-suicidal group,the suicidal group had a higher proportion of females and comorbid medical conditions.Additionally,serum IgA and testosterone(TES)levels were higher in the suicidal group(all P<0.05).Multivariate Logistic regression analysis revealed significant positive associations between IgA,comorbid medical conditions,and suicidal ideation and behavior.Conclusion Elevated serum IgA levels and the presence of comorbid medical conditions are risk factors associated with suicidal ideation and behavior in patients with bipolar depression episodes.This finding contributes to more accurate risk identification and effective intervention strategies.
