自拟清肝方联合水飞蓟宾胶囊治疗酒精性肝病的疗效观察

摘　　要：目的:观察分析卢秉久教授运用自拟清肝方联合水飞蓟宾胶囊(水林佳)治疗酒精性肝病的临床疗效。方法:采用回顾性分析的研究方法将就诊于辽宁中医药大学附属医院卢秉久教授门诊的168例酒精性肝病湿热内蕴型的患者,按照治疗方案,分成治疗组和对照组,每组84例。治疗组采用自拟清肝方(300ml/次,2次/d,饭后30min服用),另配合水飞蓟宾胶囊(3粒/次,2次/d,口服)治疗,对照组用水飞蓟宾胶囊(3粒/次,2次/d,口服)治疗,两组疗程均为4周。治疗过程中均戒酒。分别观察治疗前后两组患者的肝功能丙氨酸氨基转移酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、谷氨酰转肽酶(GGT)、白蛋白(ALB)、球蛋白(GLOB)、碱性磷酸酶(ALP)水平以及中医症状量化积分的变化。比较两组患者的临床疗效及安全性。结果:(1)自拟清肝方联合水飞蓟宾胶囊在改善酒精性肝病患者的肝功能方面(ALT、AST、TBIL、GGT)较单纯应用水飞蓟宾胶囊效果更好,差异有统计学意义(P<0.05);(2)自拟清肝方联合水飞蓟宾胶囊在改善胁肋胀痛、大便秘结等症状方面较单纯应用水飞蓟宾胶囊效果更好,两组比较,差异有统计学意义(P<0.05);(3)治疗组显效率为82.1%,总有效率为94.1%;对照组显效率为64.2%,总有效率为84.5%,两组间总有效率比较,差异有统计学意义(P<0.05)。结论:自拟清肝方联合水飞蓟宾胶囊在改善酒精性肝病患者的肝功能、减轻临床症状上优于单纯应用水飞蓟宾胶囊,且安全性好。Objective:To observe and analyze the clinical efficacy of Professor Lu Bingjiu's self-prepared Qinggan Formula combined with silymarin capsules(Shuilinjia)in the treatment of alcoholic liver disease.Methods:Through retrospective analysis method,a total of 168 patients with alcoholic liver disease with retention of damp-heat in the interior in the outpatient department of Professor Lu Bingjiu in Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were divided into treatment group and control group according to the treatment plan,with 84 cases in each group.The patients in the treatment group were treated with self-prepared Qinggan Formula(300 ml/one time,2 times/d,taken 30 minutes after meals)and silymarin capsules(3 capsules/one time,2 times/d,taken orally),and the patients in the control group were treated with silymarin capsules(3 capsules/one time,2 times/d,taken orally).Both groups were treated for 4 weeks.All patients stopped drinking during the treatment.The liver function levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),glutamyl transpeptidase(GGT),albumin(ALB),globulin(GLOB),alkaline phosphatase(ALP)and the quantitative score of TCM symptoms were observed before and after treatment.The clinical efficacy and safety of the two groups were compared.Results:(1)Self-prepared Qinggan Formula combined with silymarin capsules had a better effect on improving liver function(ALT,AST,TBIL and GGT)than silymarin capsules alone in patients with alcoholic liver disease,and the differences were statistically significant(P<0.05);(2)self-prepared Qinggan Formula combined with silymarin capsules was better than silymarin capsules alone in improving the symptoms of distending pain in hypochondrium and constipation,and the difference was statistically significant(P<0.05);(3)the marked effective rate of the treatment group was 82.1%,and the total effective rate was 94.1%.In the control group,the marked effective rate was 64.2%,and the total effective rate was
