德州市青少年烟草使用影响因素与控烟干预对策研究

摘　　要：目的分析德州市青少年烟草使用的影响因素,为制定有针对性的控烟干预对策提供依据。方法采取随机整群抽样方法,在德州市所有县(市、区)的初中、高中(职高)进行抽样,选择其中部分班级的学生进行调查;采用二分类变量logistic回归分析青少年烟草使用影响因素。结果多因素分析结果显示,男生更容易吸烟(OR=1.93,95%CI:1.19~3.13)。关于烟草广告暴露问题,零售点烟草广告与吸烟行为相关(OR=2.48,95%CI:1.54~3.99);有吸烟的朋友与吸烟行为高度相关(OR=6.60,95%CI:3.31~13.16);使用过电子烟与吸烟行为相关(OR=6.43,95%CI:4.12~10.03)。结论德州市青少年吸烟行为受到烟草危害认知、烟草广告暴露、周围人员是否吸烟等多种因素影响,学校、家庭和社会需要联动配合,共同构建无烟环境,引导青少年远离烟草。Objective To analyze the influencing factors of adolescent tobacco use in Dezhou City,and provide evidence-based medical evidence for the development of targeted interventions.Methods Through random cluster sampling,middle and high schools(vocational high schools) in all counties and districts of Dezhou City were sampled.Students in selected classes at these schools were surveyed.Binary logistic regression were used to analyze the influencing factors of tobacco use among adolescents.Results A total of 4 705 students from 33 schools were included in the survey.The smoking rate of male students and the use rate of electronic cigarette were higher than those of female students.The smoking rate and electronic cigarette use rate of vocational high school students were higher than those of junior high school and senior high school students.Places were common places for teenagers to smoke.The results of multivariate analysis show that male students were more likely to smoke,and those students who lack awareness of the dangers of smoking were more likely to smoke(OR=2.48,95% CI:1.54~3.99).With regard to tobacco advertising exposure,there was only a correlation between retail tobacco advertising and smoking behavior(OR=2.48,95% CI:1.54~3.99).Having friends who smoked was highly correlated with smoking behavior(OR=6.60,95% CI:3.31~13.16),and e-cigarette use was associated with smoking behavior(OR=6.43,95%CI:4.12~10.03).Conclusion The smoking behavior of teenagers in Dezhou City is affected by many factors,such as the awareness of tobacco hazards,exposure to tobacco advertisements,and whether the surrounding people smoke.Schools,families and society need to cooperate to jointly build a smoke-free environment and guide teenagers to stay away from tobacco.
