摘　　要：目的探讨尿液细胞学巴黎报告系统对尿路上皮细胞肿瘤的诊断价值。方法回顾性研究尿脱落细胞学标本110例,由细胞病理学医生采用双盲法分别用传统报告模式和巴黎报告系统进行评审和重新分类,以手术病理(活检)诊断结果为“金标准”,比较哪种诊断模式与手术病理(活检)诊断有更强的相关性。结果110例中,患者平均年龄58.8岁。男女比例为4.2:1。尿液细胞学巴黎报告系统对高级别尿路上皮细胞肿瘤诊断的敏感性(96.7%)和诊断符合率(93.6%)均要明显优于传统细胞学的敏感性(78.3%)和诊断符合率率(80.8%),差异有统计学意义。与传统尿液细胞学诊断相比,巴黎报告系统对非典型细胞的报告率从23.6%下降到6.4%。而诊断特异性在两者间差异无统计学意义。结论泌尿细胞学巴黎报告系统通过规范性的诊断标准和术语提高了细胞学在尿路上皮细胞肿瘤的诊断效能,值得推广应用。Objective To find out the value of the Paris System(TPS) for reporting urinary cytology in the diagnosis of urothelial neoplasms and compare it with the traditional reporting system currently used in our laboratory.Methods This retrospective study was undertaken on 110 urine cytology cases.The cytopathologists were blindly reviewed and reclassified according to the traditional reporting system and the Paris System,respectively.The traditional reporting system and TPS were compared with the corresponding histological diagnosis to determine which correlated better.Results A total of 110 cases were meticulously reviewed.The mean age of the patients was 58.8 years.The ratio of male to female was 4.2:1.The sensitivity was 96.7% and diagnostic accuracy was 93.6% for detecting HGUC of TPS were higher than that of traditional cytology that were 78.3% and 80.8%,and there were statistically significant differences.The reporting rate of atypical cells in the Paris reporting system decreased from 23.6% to 6.4% compared with traditional urine cytological diagnosis.But there was no statistically significant difference in specificity.Conclusion The Paris Report System has improved the diagnostic efficiency of cytology in urothelial cell tumors by standardizing the diagnostic criteria and terminology,and is worthy of promotion and application in the system of reporting for urine cytology.
