摘  要:目的:探讨MDM2基因第309位单核苷酸多态性(SNP309)与白血病发病风险之间的相关性。方法:通过检索英文和中文数据库获取文献。根据纳入和排除标准筛选,提取数据,采用Stata 12.0软件进行Meta分析,使用优势比OR及其95%可信区间(CI)来评估五种遗传模型的关联;使用假阳性报告概率(FPRP)、贝叶斯错误发现概率(BFDP)和Venice标准来评估具有统计学意义关联的可信度。结果:本研究共纳入16项病例对照研究,其中病例组2 900人,对照组6 876人。Meta分析结果显示,MDM2 SNP309多态性增加了亚洲人群白血病的发病风险(GG vs TT:OR=2.57;GT vs TT:OR=1.57;GG+GT vs TT:OR=1.78;GG vs GT+TT:OR=1.98;G vs T:OR=1.59),亚组分析结果显示,成人组、儿童与成人混合组、健康对照、匹配研究、非匹配研究以及急性髓系白血病(AML)中观察到MDM2突变增加白血病发病风险。排除低质量和HWD研究后进行敏感性分析,发现MDM2多态性与白血病发病显著相关。但此次Meta分析显示为“低可信度”。结论:MDM2 SNP309多态性与白血病发病风险显著相关,但其可信度分析显示为低可信度,其多态性与白血病发病的关联可能是由于假阳性结果所致。Objective:To investigate the correlation between the 309th single nucleotide polymorphism(SNP309)of MDM2 gene and the risk of leukemia.Methods:Literature was obtained by searching English and Chinese databases.According to the inclusion and exclusion criteria,the Meta were extracted,and the meta-analysis was carried out by Stata 12.0 software.The odds ratio OR and its 95%confidence interval(CI)were used to evaluate the association of five genetic models.In addition,false positive reporting probability(FPRP),Bayesian error detection probability(BFDP)and Venice standard are used to evaluate the reliability of statistically significant correlation.Results:Sixteen case-control studies were included in this study,including 2900 cases and 6876 controls.Meta-analysis showed that MDM2 SNP309 polymorphism increased the risk of leukemia in Asian population(GG vs TT:OR=2.57;GT vs TT:OR=1.57;GG+GT vs TT:OR=1.78;GG vs GT+TT:OR=1.98;G vs T:OR=1.59).Subgroup analysis showed that MDM2 mutation increased the risk of leukemia in adult group,mixed group of children and adults,healthy control,matched study,mismatched study and acute myeloid leukemia(AML).After excluding low-quality and HWD studies,sensitivity analysis showed that MDM2 polymorphism was significantly related to leukemia.However,this meta-analysis shows"low credibility".Conclusion:The polymorphism of MDM2 SNP309 is significantly related to the risk of leukemia,but its reliability analysis shows that it is low,and the correlation between its polymorphism and leukemia may be caused by false positive results.
