摘 要:目的 探究急性感染性腹泻患者血清白细胞介素-1β(interleukin-1β,IL-1β)、白细胞介素-17(interleukin-17,IL-17)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平与肠道菌群的相关性。方法 选取148例急性感染性腹泻患者作为研究组,148例健康体检者作为对照组。对比研究组和对照组血清IL-1β、IL-17、TNF-α水平,并比较研究组不同血清IL-1β、IL-17、TNF-α水平下的肠道菌群,采用Pearson分析研究组血清IL-1β、IL-17、TNF-α水平与肠道菌群的相关性,通过受试者操作特征(receiver operating characteristic, ROC)曲线分析血清IL-1β、IL-17、TNF-α水平诊断急性感染性腹泻的价值。结果 研究组的血清IL-1β、IL-17、TNF-α水平高于对照组,差异有统计学意义(t=27.405、11.562、11.390,P<0.05);血清IL-1β、IL-17和TNF-α联合诊断急性感染性腹泻的曲线下面积(area under the curve, AUC)为0.876,大于各指标单一诊断;急性感染性腹泻患者血清IL-1β、IL-17、TNF-α水平与双歧杆菌、乳酸菌呈负相关(r=-0.527、-0.613、-0.594、-0.536、-0.621、-0.589,P<0.05),与肠杆菌呈正相关(r=0.074、0.082、0.089,P<0.05)。结论 急性感染性腹泻患者的血清IL-1β、IL-17、TNF-α水平升高,与患者肠道菌群含量存在密切相关性,且在急性感染性腹泻诊断方面有一定应用价值。Objective To investigate the correlation between the levels of interleukin-1β(IL-1β),interleukin-17(IL-17),and tumor necrosis factor-α(TNF-α)in serum and intestinal flora in patients with acute infectious diarrhea.Methods A total of 148 patients with acute infectious diarrhea were selected as the study group,and 148 healthy individuals undergoing physical examinations were selected as the control group.The levels of serum IL-1β,IL-17,and TNF-αin the study group and control group were compared,and the intestinal flora under different serum IL-1β,IL-17,and TNF-αlevels in the study group were also compared.Pearson analysis was used to analyze the correlation between serum IL-1β,IL-17,and TNF-αlevels and intestinal flora in the study group.The receiver operating characteristic(ROC)curve analysis was used to analyze the value of serum IL-1β,IL-17,and TNF-αlevels in the diagnosis of acute infectious diarrhea.Results The levels of serum IL-1β,IL-17,and TNF-αin the study group were higher than those in the control group,and the dfferences were statistically significant(t=27.405,11.562,11.390,P<0.05).The area under the curve(AUC)of the combined diagnosis of IL-1β,IL-17,and TNF-αin serum for acute infectious diarrhea was O.876,which was greater than the AUCs of each individual diagnosis.The levels of serum IL-1β,IL-17,and TNF-αin patients with acute infectious diarrhea were negatively correlated with Bifidobacterium and lactic acid bacteria(r=-0.527,-0.613,-0.594,-0.536,-0.621,-0.589,P<0.05),and positively correlated with Enterobacteria(r=0.074,0.082,0.089,P<0.05).Conclusion The levels of serum IL-1β,IL-17,and TNF-αin patients with acute infectious diarrhea are increased,which closely correlates with the content of intestinal flora in patients,and it holds certain application value in the diagnosis of acute infectious diarrhea.
