腹腔镜卵巢囊肿剥除术中不同止血方法对卵巢功能的影响比较

摘　　要：目的 探究腹腔镜卵巢囊肿剥除术中不同止血方法对卵巢功能的影响。方法 选取150例在本院住院行腹腔镜卵巢囊肿剥除术患者,将其根据止血方法不同分为研究组与对照组,每组75例。研究组予以缝合止血法,对照组予以电凝止血法。比较两组术前及术后3个月雌二醇(E_(2))、卵泡刺激素(FSH)、卵巢窦卵泡(Fo)数量、卵巢最大平面平均直径(MOD)。结果 两组患者术后E_(2)水平均较术前下降,FSH水平升高,且研究组E_(2)、FSH水平分别为(122.81±33.25)pmol/L、(9.25±1.52)IU/L,较对照组的(107.69±30.14)pmol/L、(13.56±1.50)IU/L变化幅度小,差异有统计学意义(P<0.05)。两组术后对侧Fo数量和MOD比较,差异无统计学意义(P>0.05);两组患者术后术侧Fo数量和MOD比较,差异具有统计学意义(P<0.05)。结论 对腹腔镜卵巢囊肿剥除术患者而言,选择缝合止血法可最大限度地保护卵巢组织,减轻对卵巢功能的破坏,避免卵巢功能衰退。Objective To explore the effect of different hemostatic methods on ovarian function during laparoscopic ovarian cyst removal.Methods 150 patients who underwent laparoscopic ovarian cyst removal in our hospital were divided into study group and control group according to different methods of hemostasis,with 75 cases in each group.The study group was treated with suture hemostasis,and the control group was treated with electrocoagulation hemostasis.Estradiol(E_(2)),follicle-stimulating hormone(FSH),number of ovarian sinus follicles(Fo) and maximum mean plane diameter(MOD) of ovary were compared between the two groups before and 3 months after operation.Results After operation,the level of E_(2) in both groups decreased and the level of FSH increased,and the levels of E_(2) and FSH in the study group were(122.81±33.25) pmol/L and(9.25±1.52) IU/L,respectively.Compared with the control group,the changes of(107.69±30.14) pmol/L and(13.56±1.50) IU/L were small,and the difference was statistically significant(P<0.05).There was no significant difference in contralateral Fo quantity and MOD between the two groups(P>0.05).There were significant differences in the number of Fo and MOD between the two groups(P<0.05).Conclusion For patients with laparoscopic ovarian cyst removal,suture hemostasis can protect ovarian tissue to the maximum extent,reduce the destruction of ovarian function,and avoid ovarian function decline.
