摘 要:目的系统评价全身振动训练对脑卒中偏瘫患者平衡及运动功能的影响。方法计算机检索The Cochrane Library、PubMed、EMbase、Web of Science、中国知网、万方数据、维普网、中国生物医学文献数据库,搜索有关全身振动训练在脑卒中偏瘫患者中应用的随机对照试验(RCT),检索时间均为建库至2023年3月25日。由2名研究者独立按照纳入和排除标准筛选文献、评价纳入研究的偏倚风险并从中提取数据,采用RevMan 5.4软件进行Meta分析。结果共纳入文献17篇,涉及867例脑卒中偏瘫患者。Meta分析结果显示,试验组BBS评分(MD=5.93,95%CI 2.98~8.88,P<0.0001)、Fugl-Meyer评定量表上肢部分(MD=8.55,95%CI 5.17~11.92,P<0.00001)、Fugl-Meyer评定量表下肢部分(MD=4.72,95%CI 2.51~6.94,P<0.0001)、改良Bathel指数(MD=12.04,95%CI 6.32~17.77,P<0.0001)、MAS(MD=-0.4,95%CI-0.49~-0.30,P<0.00001)评分均明显优于对照组;两组Bathel指数(MD=9.31,95%CI-2.21~20.83,P=0.11)无显著性差异。结论现有证据表明,全身振动训练可改善脑卒中偏瘫患者平衡及运动功能,也可降低患者的肌张力。Objective To systematically evaluate the effects of whole body vibration training on balance and motor function in stroke patients with hemiplegia.Methods The Cochrane Library,PubMed,EMbase,Web of Science,CNKI,Wanfang Data,VIP,and Chinese biomedical literature database electronically searched to collect randomized controlled trials(RCTS)on the application of whole body vibration training in stroke patients with hemiplegic disease.The retrieval time was from March 25,2023.Two researchers independently screened literature according to inclusion and exclusion criteria,evaluated the bias risk of included studies,and extracted data from them.RevMan 5.4 software was used for meta-analysis.Results A total of 17 literatures were included,involving 867 stroke patients with hemiplegia.The results of meta-analysis showed that BBS score(MD=5.93,95%CI 2.98~8.88,P<0.0001),upper limb of Fugl-Meyer rating scale(MD=8.55,95%CI 5.17~11.92,P<0.00001),Fugl-Meyer rating scale lower extremity(MD=4.72,95%CI 2.51~6.94,P<0.0001),improved Bathel index(MD=12.04,95%CI 6.32~17.77,P<0.0001),MAS(MD=0.4,95%CI 0.49~0.30,P<0.00001),score were significantly better than the control group.Bathel index(MD=9.31,95%CI-2.21~20.83,P=0.11)had no significant difference between the two groups.Conclusion Current evidence suggested that whole-body vibration training can improve balance and motor function and reduce muscle tone in stroke patients with hemiplegia.
