摘　　要：目的 探讨同型半胱氨酸(Hcy)、低氧诱导因子-1 α (HIF-1 α)对进展性脑梗死(PCI)患者预后的预测价值。方法 选取2020年1月至2022年11月南京大学医学院附属苏州医院收治的急性脑梗死(ACI)患者178例,其中PCI患者95例为进展组,非PCI患者83例为非进展组。根据治疗90d时预后将PCI患者分为预后良好组64例和预后不良组31例。检测各组血清Hcy、HIF-1α水平,分析两项因子与梗死体积、NIHSS的相关性,绘制ROC曲线评价Hcy、HIF-1α对PCI患者预后的评估价值。结果 进展组中易损斑块比例、梗死体积、NIHSS评分高于非进展组,血清Hcy、HIF-1 α水平也高于非进展组(均P<0.05)。进展组患者血清Hcy及HIF-1 α水平与梗死体积、NIHSS评分呈正相关(均P<0.05)。预后不良组患者血清Hcy、HIF-1α水平明显高于预后良好组(均P<0.05)。Hcy联合HIF-1α预测PCI患者预后的曲线下面积为0.825,联合预测敏感度高于单一指标(P<0.05)。结论 血清Hcy、HIF-1 α水平与PCI患者病情密切相关,检测血清Hcy、HIF-1α水平有助于早期评估PCI患者预后。Objective To investigate the prognostic value of homocysteine(Hey)and hypoxic-inducible factor-1α(HIF-1α)in patients with progressive cerebral infarction(PCI).Methods A total of 178 patients with acute cerebral infarction(ACI)admitted to Suzhou Hospital Affiliated to Nanjing University School of Medicine from January2020 to November 2022 were selected,of which 95 patients with PCI were in the progressive group and 83 patients with non-PCI were in the non-progressive group.According to the prognosis after 90 days of treatment,PCI patients were divided into good prognosis group(64 cases)and poor prognosis group(31 cases).The levels of serum Hcy and HIF-1αwere detected in each group.The correlation between the two factors and infarct volume and NIHSS was analyzed,and ROC curve was drawn to evaluate the prognostic value of Hcy and HIF-1αin patients with PCI.Results The proportion of vulnerable plaque,infarct volume and NIHSS score in the progressive group were higher than the nonprogressive group,and the serum levels of Hcy and HIF-1αwere also higher than non-progressive group(~(all)P<0.05).The levels of serum Hcy and HIF-1αwere positively correlated with infarct volume and NIHSS score in advanced group(~(all)P<0.05).The levels of serum Hcy and HIF-1αin poor prognosis group were significantly higher than good prognosis group(~(all)P<0.05).The area under the curve of Hcy combined with HIF-1αpredicted the prognosis of PCI patients was0.825,and the combined prediction sensitivity was higher than that of single index(P<0.05).Conclusion The levels of serum Hcy and HIF-1αare closely related to the condition of patients with PCI,and detection of serum Hcy and HIF-1αlevels is helpful for early prognosis assessment of patients with PCI.
