摘 要:目的:观察加味甘草泻心汤含漱治疗乳腺癌化疗相关口腔黏膜炎的疗效。方法:选择101例乳腺癌化疗相关口腔黏膜炎患者,按随机数字表法分为对照组50例和观察组51例。2组均给予紫杉类化疗方案化疗,对照组给予康复新液含漱,观察组给予加味甘草泻心汤含漱。评价2组临床疗效,比较2组口腔黏膜愈合时间、疼痛评分、口腔pH值、中医证候评分、口腔微生物数量及炎症因子水平。结果:观察组总有效率为98.04%,高于对照组82.00%(P<0.05)。治疗后,2组视觉模拟评分法(VAS)评分、口腔pH值、各项中医证候评分较治疗前降低(P<0.05);观察组VAS评分、口腔pH值、各项中医证候评分低于对照组(P<0.05),观察组口腔黏膜愈合时间短于对照组(P<0.05)。治疗后,2组口腔链球菌、韦荣球菌、奈瑟菌数量较治疗前增加(P<0.05);且观察组口腔链球菌、韦荣球菌、奈瑟菌数量多于对照组(P<0.05)。治疗后,2组血清乏氧诱导因子-1α(HIF-1α)、白细胞介素-6(IL-6)、转化生长因子-β1(TGF-β1)、肿瘤坏死因子-α(TNF-α)水平较治疗前降低(P<0.05);且观察组HIF-1α、IL-6、TGF-β1、TNF-α水平低于对照组(P<0.05)。治疗后,2组中性粒细胞、白细胞计数较治疗前增加(P<0.05);且观察组中性粒细胞、白细胞计数高于对照组(P<0.05)。结论:加味甘草泻心汤含漱治疗乳腺癌化疗相关口腔黏膜炎,可改善口腔微生物数量,抑制机体炎症,提升中性粒细胞、白细胞计数,改善口腔环境,促进口腔黏膜愈合,缓解患者疼痛,提升临床疗效。Objective:To observe the curative effect of gargle with Modified Gancao Xiexin Decoction for chemotherapy-related oral mucositis.Methods:A total of 101 cases of patients with breast cancer chemotherapy-related oral mucositis were divided into the control group and the observation group according to the random number table method,with 50 and 51 cases in each group respectively.Both groups were given taxane chemotherapy;the control group was given gargle with Kangfuxin Liquid,and the observation group was given gargle with Modified Gancao Xiexin Decoction.The clinical effects in the two groups were evaluated;the healing time of oral mucosa,pain scores,oral pH values,traditional Chinese medicine(TCM) syndrome scores,the quantity of oral microorganisms and the levels of inflammatory factors were compared between the two groups.Results:The total effective rate was98.04% in the observation group,higher than that of 82.00% in the control group(P<0.05).After treatment,Visual Analogue Scale(VAS) scores,oral pH values,and TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05);VAS score,oral pH value and TCM syndrome score in the observation group were lower than those in the control group(P<0.05),and the healing time of oral mucosa in the observation group was shorter than that in the control group(P<0.05).After treatment,the quantity of oral Streptococcus,Veillonella,and Neisseria in the two groups was increased when compared with that before treatment(P<0.05).The quantity of oral Streptococcus,Veillonella,and Neisseria in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of hypoxia inducible factor-1α(HIF-1α),interleukin-6(IL-6),transforming growth factor-β1(TGF-β1),and tumor necrosis factor-α(TNF-α) in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of HIF-1α,IL-6,TGF-β1,and TNF-α in the observation group were lower than those in the control group(P<0.05).After
