十字形分牙法与常规微创拔牙法在下颌阻生齿拔除术中的应用效果比较

摘　　要：目的探讨十字形分牙法与常规微创拔牙法在下颌阻生齿拔除术中的应用效果差异。方法选取2021年1月至2023年1月在河南省直第三人民医院行下颌阻生齿拔除术治疗的66例下颌阻生齿患者为研究对象,按照随机数字表法分为观察组(33例)与对照组(33例),观察组患者行十字形分牙法拔除牙齿,对照组行常规微创拔牙法拔除牙齿。比较两组患者术中疼痛程度[视觉模拟评分法(VAS)]、牙齿拔除时间、术后出血持续时间、术后张口受限程度、术后面部肿胀程度及术后并发症发生情况。结果观察组术中VAS评分低于对照组,牙齿拔除时间及术后出血持续时间均短于对照组(P<0.05)。观察组术后1、3 d时张口受限程度和面部肿胀程度均比对照组轻(P<0.05)。两组术后邻牙损伤、颞下颌关节疼痛及干槽症并发症总发生率比较,观察组低于对照组(P<0.05)。结论与微创拔牙法相比,十字形分牙法在下颌阻生齿拔除术中应用效果更显著,能有效减少术中创伤,缩短手术时间,减轻术后张口受限及面部肿胀程度,降低并发症发生率,值得推广。Objective To compare the application effect of the cross-shaped tooth extraction and conventional minimally invasive tooth extraction for impacted mandibular teeth.Methods Sixty-six patients with impacted mandibular teeth underwent tooth extraction in the Third People’s Hospital of Henan Province from January 2021 to January 2023 were selected,and randomly assigned into two groups,each with 33 cases.Observation group received cross-shaped tooth extraction,while control group adopted conventional minimally invasive tooth extraction.Then the degree of intraoperative pain[visual analogue scale(VAS)],time of tooth extraction,duration of postoperative bleeding,degree of postoperative mouth opening limitation,degree of postoperative facial swelling and the occurrence of postoperative complications were compared between two groups.Results Observation group had lower intraoperative VAS score,shorter tooth extraction time and less postoperative bleeding compared with control group(P<0.05).The degree of mouth opening limitation and facial swelling was milder in observation group than that of control group at postoperative 1 and 3 days(P<0.05).The total incidence rate of postoperative complications such as adjacent tooth injury,temporomandibular joint pain and dry socket in observation group was lower than that in control group(P<0.05).Conclusion Compared with the minimally invasive extraction method,cross-shaped tooth extraction is more effective in impacted mandibular teeth extraction,which has the advantages of mild intraoperative trauma,short surgical time,mild degrees of postoperative mouth opening limitation and facial swelling and low complication rate,so it is worth promoting.
