基于“湿热致病”理论探讨清肠化湿法治疗溃疡性结肠炎的临床疗效及作用机制

摘　　要：基于“湿热致病”的理论内涵,认为大肠湿热丛生,蕴结肠道,导致热盛肉腐、气血阻滞是溃疡性结肠炎(ulcerative colitis, UC)形成的关键。中医认为UC病位在肠,以湿热为标,证候以大肠湿热证为主,治则以清利大肠为纲。在临床应用上清肠化湿类方剂治疗UC效果显著,能够明显改善患者症状,且具安全性高、不良反应小等特点。现代药理学研究发现清肠化湿类方剂可通过调节肠道菌群、修复肠黏膜屏障、维持肠道免疫平衡、减轻肠道炎症、缓解氧化应激等途径发挥治疗作用。笔者通过整理文献,对近年来清肠化湿方治疗UC的临床疗效和作用机制进行了系统的总结分析,并就其不足提出建议,以期为阐释清肠化湿法治疗UC的科学内涵提供科学依据,为评价和挖掘经典中药及其复方治疗UC的潜力提供思路。Based on the theoretical connotation of"dampness and heat causing disease",it is believed that the accumulation of dampness and heat in the intestine,which leads to heat accumulation and polyp decay,as well as qi and blood blockage,is the key to the formation of ulcerative colitis(UC).Traditional Chinese medicine believes that UC is locate in the intestine,characterized by dampness and heat.The main syndrome is dampness and heat in the large intestine,and the treatment principle is to clear and benefit the large intestine.In clinical application,the Qingchang Huashi Compound has a significant therapeutic effect on UC,can significantly improve patient symptoms,and has high safety and minimal adverse reactions.Modern pharmacological research has found that clearing the intestines and removing dampness can exert therapeutic effects by regulating intestinal microbiota,repairing intestinal mucosal barrier,maintaining intestinal immune balance,reducing intestinal inflammation,and alleviating oxidative stress.The author has systematically summarized and analyzed the research results and mechanism of Qingchang Huashi Compound in the treatment of UC in recent years by organizing literature,and proposed suggestions for the shortcomings.The aim is to provide scientific basis for explaining the scientific connotation of Qingchang Huashi Compound in the treatment of UC,and to provide reference for evaluating and exploring the potential of classical Chinese medicine and its compound formulas in the treatment of UC.
