内镜止血在急性非静脉曲张性上消化道出血临床治疗中的效果分析

摘　　要：目的 分析在临床急性非静脉曲张性上消化道出血(ANVUGIB)治疗中应用内镜止血的效果.方法 选取2021年4月-2023年4月在昭通市第一人民医院就诊的ANVUGIB患者95例,按照是否予以内镜止血治疗将其分为对照组(n=40)和观察组(n=55),其中对照组行常规药物治疗,观察组则行消化内镜下止血治疗.记录两组患者的止血效果、再出血情况以及临床各指标水平,并比较治疗前、3d后两组炎性指标水平的差异.结果 观察组的止血总有效率为90.91%,高于对照组的75.00%(P<0.05),且观察组的再出血率为1.82%,较对照组的15.00%更低(P<0.05).治疗3d后,观察组CRP、IL-4、IL-6以及TNF-α水平均低于对照组(P<0.05).在出血停止、肠鸣音恢复以及住院时间比较上,观察组短于对照组(P<0.05),其输血量也少于对照组(P<0.05).结论 内镜止血能够有效帮助ANVUGIB患者止血,降低再出血风险,减轻患者的炎症反应,加速其恢复,值得临床推广.Objective To analyze the clinical efficacy of endoscopic hemostasis in the treatment of acute non variceal upper gastrointestinal bleeding(ANVUGIB).Method A retrospective analysis was conducted on the clinical data of 95 patients with ANVUGIB admitted to our hospital from April 2021 to April 2023.They were divided into a control group(n=40)and an observation group(n=55)based on whether endoscopic hemostasis treatment was given.The control group received conventional drug conservative treatment,while the observation group received endoscopic hemostasis treatment.Record the hemostatic effect,rebleeding status,and clinical indicators of the two groups of patients,and compare the differences in inflammatory index levels between the two groups before and 3 days after treatment.Result The hemostatic effective rate of the observation group was 90.91%,higher than 75.00%of the control group(P<0.05),and the rebleeding rate of the observation group was 1.82%,lower than 15.00%of the control group(P<0.05).After 3 days of treatment,the observation group showed the levels of CRP,IL-4,IL-6,and TNF-αwere lower than those of the control group(P<0.05).In terms of cessation of bleeding,recovery of bowel sounds,and comparison of hospitalization time,the observation group was shorter than the control group(P<0.05),and its blood transfusion volume was also less than the control group(P<0.05).Conclus ion Endoscopic hemostasis can effectively help ANVUGIB patients stop bleeding,reduce the risk of rebleeding,alleviate inflammation,accelerate recovery,and worth promoting in clinical practice.
