摘  要:目的:探究DNA倍体定量分析技术在口腔鳞状细胞癌筛查中的应用及效果评价。方法:2022年3月~2023年6月,对西安交通大学口腔医院门诊及住院107例患者,进行DNA倍体筛查及液基细胞学检查。DI>2.5的患者,为阳性患者,进行病理组织活检;DI<2.5的患者,若无明确临床恶性指征,视为阴性。将患者DNA倍体筛查结果与液基细胞学结果及病理活检结果进行比较。结果:107例患者中,28例患者进行了病理检查,其中27例患者DNA倍体诊断意见与病理报告结果相符,吻合率为96.43%。DI>2.5且病理结果为恶性的患者共24例,DNA倍体诊断意见与病理报告结果相符,吻合率为100%;DI>2.5,病理结果未明确恶性的1例,为鳞状上皮乳头状增生,部分上皮中-重度不典型增生;D1<2.5,病理结果为良性的患者2例;病理结果为恶性的患者1例,显示有效检测细胞不足。口腔鳞状细胞癌共22例,21例DNA倍体诊断意见与病理报告结果相符,吻合率为95.45%。结论:对于口腔鳞状细胞癌,DNA倍体筛查结果与病理活检结果的吻合率较高,DNA倍体定量分析比传统的细胞学检查敏感,是一种较为可靠、客观的口腔癌无创筛查方法.Objective:To explore the application and effectiveness evaluation of DNA ploidy quantitative analysis technology in screening oral squamous cell carcinoma.Methods:From March 2022 to June 2023,DNA ploidy screening and liquid based cytology examinations were performed on 107 outpatient and inpatient patients at the Stomatological Hospital of Xi'an Jiaotong University.Patients with DI>2.5 were considered positive and undergo pathological tissue biopsy,with DI<2.5 considered negative if there are no clear clinical malignant indications.The results of DNA ploidy screening were compared with the results of liquid based cytology and pathological biopsy.Results:Am ong the 107 patients,28 underwent pathological examination,and 27 of them had DNA ploidy diagnosis opinions that were consistent with the pathological report results,with a coincidence rate of 96.43%.A total of 24 patients with DI>2.5 and pathological results were malignant.The diagnostic opinion of DNA ploidy was consistent with the pathological report results,with a coincidence rate of 100%.One case with DI>2.5 and no clear malignancy in pathological results was squamous papillary hyperplasia,with some epithelial moderate to severe atypical hyperplasia.Two patients with DI<2.5 and pathological results as benign,the pathological result showed 1 patient with malignancy,indicating effective detection of insufficient cells.There were total of 22 cases of oral squamous cell carcinoma,and the diagnostic opinion of DNA ploidy in 21 cases were consistent with the pathological report results,with a coincidence rate of 95.45%.Conclusion:For oral squamous cell carcinoma,the coincidence rate between DNA ploidy screening results and pathological biopsy results is high.DNA ploidy quantitative analysis is more sensitive than traditional cytological examinations.It is a reliable and objective non-invasive screening method for oral squamous cell carcinoma.
