血栓通注射液联合阿替普酶溶栓治疗缺血性脑卒中的效果及对脑血管血流状态、MCP-1、VE-cadherin的影响

摘　　要：目的探讨血栓通注射液联合阿替普酶溶栓治疗缺血性脑卒中的效果及对脑血管血流状态、单核细胞趋化蛋白-1(MCP-1)、血管内皮细胞钙黏蛋白(VE-cadherin)的影响。方法选取2020年3月至2022年3月我院收治的100例缺血性脑卒中患者为研究对象,以随机数字表法将其分为对照组与观察组,各50例。两组患者入院后均给予常规治疗,另对照组给予注射用阿替普酶溶栓治疗,观察组在对照组基础上加血栓通注射液治疗。比较两组的治疗效果。结果观察组的治疗总有效率为94.00%,高于对照组的78.00%(P<0.05)。治疗后,观察组的双侧大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)及左右椎动脉(VA)血流速度均高于对照组(P<0.05)。治疗后,观察组的神经元特异性烯醇化酶(NSE)水平低于对照组,神经生长因子(NGF)、脑源性神经营养因子(BDNF)水平高于对照组(P<0.05)。治疗后,观察组的MCP-1、VE-cadherin水平低于对照组(P<0.05)。结论血栓通注射液联合阿替普酶溶栓治疗缺血性脑卒中不仅能够提高临床效果,改善脑血管血流状态,还能调节MCP-1、VE-cadherin水平,促进神经功能恢复,值得推广。Objective To investigate the effect of Xueshuantong injection combined with alteplase thrombolytic therapy on ischemic stroke and its influences on cerebral vascular blood flow status,monocyte chemotactic protein-1(MCP-1)and vascular endothelial cadherin(VE-cadherin).Methods A total of 100 patients with ischemic stroke admitted in our hospital from March 2020 to March 2022 were selected as the research objects and divided into control group and observation group by random number table method,with 50 cases in each group.Both groups were given conventional treatment after admission.The control group was treated with alteplase thrombolytic therapy,and the observation group was treated with Xueshuantong injection on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 94.00%,which was higher than 78.00%in the control group(P<0.05).After treatment,the blood flow velocity of middle cerebral artery(MCA),anterior cerebral artery(ACA),posterior cerebral artery(PCA)and vertebral artery(VA)in the observation group were higher than those in the control group(P<0.05).After treatment,the level of neuron specific enolase(NSE)in the observation group was lower than that in the control group,and the levels of nerve growth factor(NGF)and brain-derived neurotrophic factor(BDNF)were higher than those in the control group(P<0.05).After treatment,the levels of MCP-1 and VE-cadherin in the observation group were lower than those in the control group(P<0.05).Conclusion Xueshuantong injection combined with alteplase thrombolytic therapy on ischemic stroke can not only improve the clinical effect,improve the blood flow status of cerebral vascular,but also regulate the levels of MCP-1 and VE-cadherin,and promote the recovery of neurological function,which is worthy of promotion.
