摘　　要：目的:探究NLRP3基因在膀胱癌患者外周血细胞及尿沉渣中的表达,并分析其与膀胱癌临床特征及预后的相关性。方法:分别收集68例非肌层浸润性膀胱癌患者(膀胱癌组)、60例良性病变者(良性病变组)和60例健康者(对照组)的血液及尿液样本,采用实时荧光定量PCR(RT-qPCR)法测定外周血单核细胞及尿沉渣中NLRP3 mRNA的表达,比较三组受试者外周血单核细胞及尿沉渣NLRP3 mRNA的表达,并用ROC曲线评价其对膀胱癌的诊断价值;并分析NLRP3的表达与膀胱癌患者临床特征及预后的相关性。结果:膀胱癌组和良性病变组患者外周血单核细胞及尿沉渣NLRP3 mRNA的表达量明显高于对照组,且膀胱癌组尿沉渣NLRP3 mRNA的表达水平明显高于良性病变组。外周血单核细胞及尿沉渣NLRP3诊断膀胱癌的AUC分别为0.754和0.922,但尿沉渣NLRP3诊断的特异性明显高于外周血单核细胞NLRP3。外周血单核细胞及尿沉渣NLRP3的表达水平与膀胱癌的分级及TNM分期具有明显的相关性(P<0.05)。外周血单核细胞NLRP3低表达组的无进展生存率为87.5%,与高表达组的70.4%相比无明显差异(P>0.05);而尿沉渣NLRP3低表达组的无进展生存率为90.5%,显著高于高表达组的66.0%(P<0.05)。结论:NLRP3在膀胱癌患者的外周血细胞及尿沉渣中高表达,与膀胱癌的临床病理特征及预后有一定的相关性,可能是膀胱癌早期诊断及复发监测的潜在肿瘤标记物。Objective:To investigate the expression of NLRP3 gene in peripheral blood cells and urinary sediment of patients with bladder cancer,and to analyze its correlation with the clinical features and prognosis of bladder cancer.Methods:Blood and urine samples were collected from 68 patients with non-muscle invasive bladder cancer(bladder cancer group),60 patients with benign disease(benign disease group)and 60 healthy subjects(control group).The expression of NLRP3 mRNA in peripheral blood mononuclear cells and urinary sediment was detected by real-time fluorescent quantitative PCR(RT-qPCR).The expression of NLRP3 mRNA in peripheral blood mononuclear cells and urinary sediment of the three groups was compared,and the ROC curve was used to evaluate its diagnostic value for bladder cancer.The correlation between the expression of NLRP3 and the clinical characteristics and prognosis of patients with bladder cancer was analyzed.Results:The expression of NLRP3 mRNA in peripheral blood mononuclear cells and urinary sediment in bladder cancer group and benign disease group was significantly higher than that in control group,and the expression of NLRP3 mRNA in urinary sediment in bladder cancer group was significantly higher than that in benign disease group.The AUC of peripheral blood mononuclear NLRP3 and urinary sediment NLRP3 in the diagnosis of bladder cancer was 0.754 and 0.922,respectively,but the specificity of urinary sediment NLRP3 diagnosis was significantly higher than that of peripheral blood mononuclear NLRP3.The expression level of NLRP3 in peripheral blood mononuclear cells and urinary sediment is significantly correlated with the grade and TNM stage of bladder cancer(P<0.05).The progression-free survival rate of the NLRP3 low expression group was 87.5%,which was not significantly different from that of the NLRP3 high expression group 70.4%(P>0.05).The progression-free survival rate of urinary sediment NLRP3 low expression group was 90.5%,which was significantly higher than 66.0%of urinary sediment NLRP3 high
