消癥止痛汤联合耳穴压豆对气滞血瘀型子宫内膜异位症痛经患者炎症因子和PGE2、PGF2α的影响

摘　　要：目的:观察消癥止痛汤联合耳穴压豆对气滞血瘀型子宫内膜异位症(EMT)痛经患者炎症因子和前列腺素E2(PGE2)和前列腺素F2α(PGF2α)的影响。方法:选取2021年3月至2022年12月期间在广州中医药大学附属中山中医院收治的98例EMT痛经患者作为研究对象。采用随机数字表法将其分为对照组(常规西医治疗,n=49)和研究组(对照组的基础上消癥止痛汤联合耳穴压豆,n=49)。对比两组疗效、疼痛情况评分[疼痛视觉模拟评分(VAS)、COX痛经症状积分、]、中医证候评分、炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)]和PGE2和PGF2α水平。结果:研究组的临床总有效率高于对照组(P<0.05)。治疗3个疗程后,研究组VAS、COX痛经症状积分、中医证候总评分低于对照组(P<0.05)。治疗3个疗程后,研究组IL-6、TNF-α、IL-8低于对照组(P<0.05)。治疗3个疗程后,研究组PGE2高于对照组,PGF2α低于对照组(P<0.05)。结论:消癥止痛汤联合耳穴压豆治疗气滞血瘀型EMT痛经患者,可有效改善痛经症状,降低中医证候评分,调节血清炎症因子和PGE2、PGF2α水平。Objective:To observe the effect of Xiaozheng zhitong decoction combined with auricular point pressing beans on inflammatory factors and prostaglandin E2(PGE2)and prostaglandin F2α(PGF2α)in patients with endometriosis(EMT)dysmenorrhea of qi stagnation and blood stasis type.Methods:98 EMT dysmenorrhea patients who were admitted to Zhongshan Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Chinese Medicine from March 2021 to December 2022 were selected as research objects.Patients were divided into control group(conventional western medicine treatment,n=49)and study group(xiaozheng zhitong decoction combined with auricular point pressing beans on the basis of control group,n=49)by random number table method.The efficacy,pain score[pain visual analogue scale(VAS),COX dysmenorrhea symptom score],chinese medicine syndrome scores,inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),interleukin-8(IL-8)]and PGE2 and PGF2αlevels were compared between two groups.Results:The total clinical effective rate in study group was higher than that in control group(P<0.05).3 courses after treatment,the VAS,COX dysmenorrhea symptom score and total chinese medicine syndrome scores in study group were lower than those in control group(P<0.05).3 courses after treatment,the levels of IL-6,TNF-αand IL-8 in study group were lower than those in control group(P<0.05).3 courses after treatment,PGE2 in study group was higher than that in control group,and PGF2αwas lower than that in control group(P<0.05).Conclusion:Xiaozheng zhitong decoction combine with auricular point pressing beans in the treatment of EMT dysmenorrhea patients with qi stagnation and blood stasis type can effectively improve dysmenorrhea symptoms,reduce TCM syndrome scores,and regulate serum inflammatory factors and PGE2 and PGF2αlevels.
