腹腔镜卵巢囊肿剔除术治疗卵巢囊肿的效果及对卵巢功能影响

摘　　要：目的:探究卵巢囊肿患者应用腹腔镜卵巢囊肿剔除术治疗的效果及对卵巢功能的影响。方法:选取2020年5月—2021年12月焦作市妇幼保健院接受卵巢囊肿剔除术治疗的130例卵巢囊肿患者作为研究对象,根据手术方式的差异将患者分为对照组(n=65,采取传统开腹手术治疗)和观察组(n=65,基于腹腔镜下手术治疗)。统计两组患者临床指标(术中失血量、手术时长、术后下床活动时间、住院总时长);两组患者均随访6个月,观察术前和术后6个月卵巢基质血流情况[血流收缩期峰速(PSV)、舒张期最低流速(EDV)],检测两组实验室指标水平[卵泡刺激素(FSH)、促黄体激素(LH)、雌激素(E_(2))]。结果:观察组术中失血量、手术时长、术后下床活动时间、住院总时长相比对照组均明显更低,差异有统计学意义(t=9.648、5.834、8.635、5.347,P<0.05);术后6个月,两组患者PSV、EDV较术前均出现显著变化,观察组较对照组变化更小,差异有统计学意义(t=4.789、7.971,P<0.05);术后6个月,两组患者FSH、LH、E_(2)较术前均出现显著变化,观察组较对照组变化更小,差异有统计学意义(t=6.632、4.186、2.065,P<0.05)。结论:腹腔镜卵巢囊肿剔除术相比传统开腹手术更具创伤小、术后恢复迅速等优势,并且对患者卵巢基质血流损伤较小,有助于卵巢储备功能恢复。Objective:To investigate the effect of applying laparoscopic ovarian cyst debridement in the treatment of patients with ovarian cysts and the effect on ovarian function.Methods:130 patients with ovarian cysts who were treated with ovarian cyst debulking surgery in the hospital from May 2020 to December 2021 were selected as the study subjects,and the patients were divided into the control group(n=65,treated with traditional open surgery) and the observation group(n=65,treated with surgery based on laparoscopy) according to the differences in surgical methods.Clinical indexes(intraoperative blood loss,duration of surgery,postoperative time to get out of bed and total length of hospitalization) were counted in both groups.Patients in both groups were followed up for 6 months,and the ovarian stromal blood flow [peak systolic velocity(PSV),diastolic minimum flow velocity(EDV)]was observed preoperatively and 6 months postoperatively,and the levels of laboratory indexes [follicle-stimulating hormone(FSH),luteinizing hormone(LH),and estrogen(E_(2))] were detected in both groups.Results:Intraoperative blood loss,duration of surgery,postoperative time out of bed,and total length of hospitalization were significantly lower in the observation group compared with the control group,and the differences were statistically significant(t=9.648,5.834,8.635,5.347;P<0.05).At6 months after surgery,both groups of patients showed significant changes in PSV and EDV compared with the preoperative period,with smaller changes in the observation group compared with the control group,and the difference was statistically significant(t=4.789,7.971;P<0.05).6 months after operation,FSH,LH,E_(2) in both groups showed significant changes compared with the preoperative period,and the changes in the observation group were smaller than those in the control group,and the difference was statistically significant(t=6.632,4.186,2.065;P<0.05).Conclusion:Laparoscopic ovarian cyst removal has the advantages of less trauma and quicker postoperative recovery
