重复经颅磁刺激联合马来酸氟伏沙明片治疗老年性抑郁症的临床疗效

摘　　要：目的探讨重复经颅磁刺激(rTIMS)联合马来酸氟伏沙明片治疗老年性抑郁症的临床疗效。方法选取2021年8月至2022年2月于赣州市第三人民医院接受治疗的80例老年性抑郁症患者作为研究对象,根据治疗方法不同分为对照组与观察组,每组40例。对照组接受马来酸氟伏沙明片治疗,观察组接受rTMS联合马来酸氟伏沙明片治疗,比较两组治疗前后不同时间汉密尔顿抑郁量表(HAMD)及艾森贝格抗抑郁症副反应量表(SERS)评分及治疗前后生命质量综合评定量表(GQOLI-74)评分和临床疗效。结果治疗1、2、4、6、8周后,观察组HAMD评分均低于对照组,差异有统计学意义(P<0.05)。两组治疗后各时间点SERS评分组内、组间比较差异无统计学意义。治疗后,观察组生活状态、社会功能、心理状况、躯体情况评分均高于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为97.50%,高于对照组80.00%,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论rTIMS联合马来酸氟伏沙明片治疗老年性抑郁症疗效确切,能有效改善患者临床症状,提高生命质量的同时具有较高的安全性,值得临床推广应用。Objective To investigate the clinical effect of repeated transcranial magnetic stimulation(rTMS)combined with fluvoxamine male-ate tablets in the treatment of senile depression.Methods 80 patients with senile depression who received treatment in Ganzhou Third People's Hos-pital from August 2021 to February 2022 were selected as the study subjects,and they were divided into the control group and the observation group according to different treatment methods,with 40 cases in each group.The control group was treated with fluvoxamine maleate tablets,the observa-tion group was treated with rTMS combined with fluvoxamine maleate tablets,the Hamilton depression scale(HAMD)at different times before and after treatmen,Asberg side-effect rating scale for antidepressant(SERS)scores at different times after treatment,and the scores of generic quality of life inventory-74(GQOLI-74)before and after treatment and clinical efficacy were compared between the two groups.Results Before treatment,there was no significant difference in HAMD scores between the two groups;HAMD scores at all time points after treatment in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in SERS scores at each time point after treatment within group,and between the two groups.After treatment,the scores of life status,social function,psychological status and physical status in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The clinical efficacy in the observation group was 97.50%,which was higher than 80.00%in the control group,and the dif-ference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion rTIMS combined with fluvoxamine maleate tablet is effective in the treatment of senile depression,can effectively improve the clinical symptoms of patients,improve the quality of life
