郴州市区反式脂肪酸对高血压影响的调查分析及其干预效果研究

摘  要:目的探讨彬州市区反式脂肪酸(TFA)对高血压可能存在的影响并分析其机制。方法2020年6月至2021年6月采用分层整群抽样方法,在郴州市区抽取5个社区发放调查问卷,并在回收的有效问卷中抽取60名居民随机分为对照组与干预组,各30名。对照组采取健康宣教干预,干预组在对照组基础上统一分发不含TFA的亚麻籽油,分发定量(150ml)的油瓶,控制每人每周用油量在原用油基础上减少30ml以上,并实施健康用油相关的具体干预措施。比较干预组干预前后重复用油情况和食用油消耗量及两组居民干预前后血压、血脂和体重指数(BMI)。结果非高血压患者322例,高血压患者185例。高血压患者与非高血压人群性别、年龄、BMI、本人或家庭成员对TFA了解情况、了解反复使用食用油会增加TFA、购物时会关注食品TFA含量、重复使用食用油行为(次)>2次/周、患有高血脂、患有心脑血管疾病占比比较差异有统计学意义(P<0.05);高血压患者与非高血压人群文化程度、了解食用油中含有TFA等指标比较差异无统计学意义。干预后,干预组食用油重复利用减少1/2、食用油重复利用≤5次/周、每周食用油消耗量≤400ml占比均高于干预前与对照组,每周食用油消耗量低于干预前与对照组,差异有统计学意义(P<0.05);对照组干预前后食用油重复利用减少1/2、食用油重复利用≤5次/周、每周食用油消耗量≤400ml、每周食用油消耗量比较差异无统计学意义。干预后,干预组收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)均低于对照组,高密度脂蛋白胆固醇(HDL-C)高于对照组,差异有统计学意义(P<0.05)。结论TFA在一定程度上可诱发或加重高血压病情,其机制可能是通过引起肥胖、血脂异常导致。Objective To explore the possible effects of trans fatty acids on hypertension and analyze its mechanism,so as to provide reference for diet health and hypertension prevention and treatment in urban residents of Chenzhou area.Methods From June 2020 to June 2021,stratified cluster sampling method was used to distribute questionnaires in 5 communities in Chenzhou urban area,and 60 residents were randomly divided into the control group and intervention group,with 30 residents in each group.The control group received health education intervention,and the intervention group distributed flaxseed oil without trans fatty acids on the basis of the control group,distributed quantitative(150 ml)oil bottles,controlled that the weekly oil consumption per person was reduced by more than 30 ml on the basis of the original oil,and implemented specific intervention measures related to health oil.The repeated oil consumption and consumption of edible oil before and after intervention,as well as the blood pressure,blood lipids and body mass index(BMI)between the two groups were compared.Results There were 322 non-hypertensive patients and 185 hypertensive patients.In people with and without hypertension,there were statistically significant differences in Gender,age,BMI,TFA knowledge of myself or family members,knowledge that repeated use of edible oil will increase TFA,attention to food TFA content when shopping,repeated use of edible oil behavior(times)>2 times/week,hyperlipidemia,cardiovascular and cerebrovascular diseases(P<0.05);there was no significant difference between the two groups in education level and knowledge of TFA in edible oil.After intervention,the proportion of edible oil reuse reduced by 1/2,edible oil reuse≤5 times/week,and weekly edible oil consumption≤400 ml in the intervention group were higher than before intervention and the control group,and weekly edible oil consumption was lower than before intervention and control group,the differences were statistically significant(P<0.05).There was no statistical
