摘　　要：不同于传统以辨证分型为主体研究肝胃疾病,叶天士从病机规律化传变角度出发,结合经典体用、性味理论,对肝胃病机从初起至传变作出详尽总结,其论肝用太过损及于胃可由郁而传为逆、积、火三端,肝胃体伤亦有阴伤、血虚、脉络不充之别。治法上,叶天士旁参张仲景、朱丹溪、缪希雍等多名医家理论经验,创立出降气和络、辛香缓消积滞等多种肝胃病特色治法,从而形成了独具一格的肝胃病证治体系。文章从肝胃病机传变规律与药物性味理论两条主线出发,总结、提炼、剖析散见于叶天士医案中的肝胃病诸多治法,并构建还原其肝胃病特色证治体系,希冀为现代肝胃疾病的治疗提供新的思路。YE Tianshi’s approach to studying liver and stomach diseases differed from traditional methods based on syndrome differentiation.He analyzed the pathogenesis regularity of these diseases,combining classical theories of body use and nature taste.YE identified how liver damage can lead to stomach problems,and the different types of injuries that can occur,such as yin injury,blood deficiency,and vein insufficiency.He drew from the experiences of other doctors to develop unique treatment methods for liver and stomach diseases,including lowering qi and collaterals,pungent incense,and relieving stagnation.This article summarizes and analyzes YE’s medical records to construct a syndrome treatment system for these diseases,with the hope of providing new ideas for modern treatment.
