基于Bobath理念下运动治疗结合呼吸引导训练对脑卒中偏瘫患者下肢功能恢复的疗效观察

摘　　要：目的研究脑卒中偏瘫患者联合采用基于Bobath理念下运动治疗、呼吸引导训练对其下肢功能恢复的效果。方法选择2022年2月至2023年4月郑州大学第一附属医院神经内科74例脑卒中偏瘫患者为研究对象,以双盲法分组标准分成对照组和治疗组。对照组(n=37)予基于Bobath理念下运动治疗,治疗组则加用呼吸引导训练。对两组下肢功能、步态能力、平衡能力和日常生活活动能力进行评估对比。结果治疗后,与对照组比较,治疗组下肢Fugl-Meyer评定量表评分[(26.10±4.94)分]增高,Wisconsin步态量表(WGS)评分[(23.04±5.07)分]降低,组间存在的差异有统计学意义(P<0.05);两组Berg平衡量表评分、日常生活活动能力(ADL)评分比较,差异亦具有统计学意义(P<0.05)。结论对脑卒中偏瘫患者应用Bobath理念下运动治疗配合呼吸引导训练作用确切,能提高下肢功能、步态能力恢复效果,还能改善平衡能力、增强日常生活自理能力,值得推荐。Objective To study the effect of Bobath based exercise therapy and breathing guided training on the recovery of lower limb function in stroke patients with hemiplegia.Methods Totally 74 patients with hemiplegia after stroke in the Department of Neurology of our hospital from February 2022 to April 2023 were selected as the study objects and di⁃vided into control group and treatment group according to the double-blind grouping standard.The control group(n=37)received exercise therapy based on Bobath concept,and the treatment group received breathing guided training.Lower extremity function,gait ability,balance ability and activities of daily living ability were evaluated and compared between the two groups.Results After treatment,compared with control group,lower limb Fugl-Meyer rating scale score[(26.10±4.94)points]increased in treatment group,Wisconsin Gait Scale(WGS)score[(23.04±5.07)points]decreased,the difference between groups was statistically significant(P<0.05).The scores of Berg balance Scale and activities of daily living(ADL)between the two groups were statistically significant(P<0.05).Conclusion Exercise therapy combined with breathing guidance training under the concept of Bobath has a definite effect on stroke patients with hemiplegia,which can improve lower limb function and gait ability recovery,improve bal⁃ance ability and enhance daily self-care ability,and is worth recommending.
