摘 要:[目的]定量检测胰腺癌(pancreatic cancer,PC)患者血清microRNA-497(miR-497)表达量,探讨其临床意义。[方法]检测70例PC患者(PC组)治疗前、30位健康者(正常对照组)血清miR-497表达量以及血清CA199、CEA含量;随访PC患者治疗后预后2年;分析血清miR-497表达量的临床价值。[结果]PC组治疗前血清miR-497表达量显著低于正常对照组(P<0.01);血清miR-497对PC的诊断敏感度显著高于血清CA199(91.0%∶76.6%,χ2=7.637,P<0.01);两者诊断准确度比较无明显差异(P>0.05),三者联合检测可提高诊断敏感性和特异性。治疗前血清miR-497表达量与PC浸润深度、淋巴结转移及是否远处转移呈负相关(均P<0.01)。治疗前血清miR-497低表达组其治疗后1年、2年生存率显著低于高表达组(P<0.05)。Cox多因素分析显示治疗前血清miR-497表达量是影响PC预后的独立高危因素。[结论]血清miR-497低表达与PC侵袭转移、不良预后密切相关,对其诊断有一定价值,可成为PC临床辅助诊断、预后评价的指标。[Objective]To quantitatively detect the expression of serum micro-RNA-497(miR-497)in patients with pancreatic cancer(PC)and to explore its clinical significance.[Methods]Real-time fluorescence quantitative RT-PCR was used to detect the expression of miR-497in serum of 70patients with PC before treatment and 30healthy controls,and the levels of CA199and CEA in serum were detected at the same time.Patients with PC were followed up for 2years after treatment.Clinical value of the expression of serum miR-497was analyzed.[Results]The expression of serum miR-497in PC group before treatment was significantly lower than that in normal control group(P<0.01).The sensitivity of serum miR-497in the diagnosis of pancreatic cancer was significantly higher than that of serum CA199(91.0%vs 76.6%,χ2=7.637,P<0.01).There was no significant difference in diagnostic accuracy between the two groups(P>0.05).The combined detection of the three items could improve the sensitivity and specificity of diagnosis.The expression of serum miR-497before treatment was negatively correlated with the depth of PC invasion,lymph node metastasis and distant metastasis(P<0.01).The 1-year and 2-year survival rate in the group with low expression of serum miR-497before treatment was significantly lower than that in the group with high expression after treatment(P<0.05).Cox multivariate analysis showed that the expression of serum miR-497before treatment was an independent high risk factor affecting the prognosis of PC.[Conclusion]The low expression of serum miR-497is closely related to the invasion,metastasis and poor prognosis of PC.It has a certain value in the diagnosis of PC,and can be used as an index for clinical auxiliary diagnosis and prognosis evaluation of PC.
