护士手部职业性接触性皮炎的影响因素分析

摘　　要：目的分析护士手部职业性接触性皮炎(OCD)的发生情况.方法选择采用手机微信提交调查问卷的2852名护士,统计职业性接触性皮炎发生情况,并开展流行病学分析.结果护士手部职业性接触性皮炎总发生率为9.85%;年龄越大、职称越高、文化程度越高、工作年限越长的护士,其OCD发生率越高;既往有过敏史、皮炎病史、临床工作每天洗手次数<10次、新冠疫情暴发后未增加洗手次数、采用其他手卫生方式、其他消毒剂及其他免洗手消毒液、工作期间每天使用洗手液洗手的次数>20次、每天佩戴一次性橡胶手套的时间>2h、工作期间每次洗手后选择不擦、经常使用护手霜及偶尔进行手部保养的护士OCD发生率较高.结论广西各级医院护士手部职业性接触性皮炎发病率较高;年龄越大、职称越高、文化程度越高、工作年限越长、洗手液洗手次数越多、一次性橡胶手套佩戴时间越长且既往有过敏史、皮炎病史的护士,其发病率越高,临床针对上述情况积极采取干预措施并加强管控,可有效控制护士手部职业性接触性皮炎的发生.Objective To analyze the incidence of occupational contact dermatitis(OCD)of hands in nurses.Methods 2852 nurses who submitted questionnaires through Wechat were selected to analyze the incidence of occupational contact dermatitis and carry out epidemiological analysis.Results The total incidence of occupational contact dermatitis in nurses was 9.85%.The higher the age,the higher the professional title,the higher the education level and the higher the working years of nurses,the higher the incidence of OCD;have a history of allergies and dermatitis,wash hands less than 10 times a day in clinical work,do not increase the frequency of hand washing after the outbreak of COVID-19,use other hand hygiene methods,use other disinfectants and other no-hand sanitizer,wash hands with hand sanitizer more than 20 times a day during work,wear disposable rubber gloves for more than 2 hours a day,choose not to wipe hands after washing their hands during work,frequently use hand cream,nurses who occasionally perform hand maintenance had a higher incidence of OCD.Conclusion The incidence of hand occupational contact dermatitis in nurses at all levels of hospitals in Guangxi is high.The older the nurses are,the higher the professional title,the higher the education level,the higher the working years,the more times they wash their hands with hand sanitizer,the longer they wear disposable rubber gloves and the history of allergies and dermatitis in the past,the higher the incidence of nurses'hand occupational contact dermatitis.In view of the above conditions,the clinical intervention measures should be taken actively and the control should be strengthened,which can effectively control the occurrence of nurses'hand occupational contact dermatitis.
