摘 要:目的 观察火刃针疗法对肩周炎模型兔肩关节组织形态学和抗氧化能力及转化生长因子(TGF)-β1/Smad的影响,探讨火刃针疗法治疗肩周炎的干预机制。方法 雄性家兔按随机数字表法分为空白组、模型组、药物组、火刃针组各8只。采用连续牵引机械运动劳损法联合冰敷刺激法建立家兔肩周炎模型,药物组及火刃针组分别行药物灌胃和火刃针治疗,于实验第33天进行实验取材,分别取肱二头肌肌腱和滑膜,苏木素-伊红(HE)染色后光镜下观察其组织形态学改变;取血清及肱二头肌、冈上肌、冈下肌、大圆肌,检测超氧化物歧化酶(SOD)和总抗氧化能力(T-AOC)及TGF-β1/Smad变化。结果 与空白组比较,模型组血清及各肌肉组织中SOD、T-AOC含量明显减少,TGF-β1、Smad1含量明显升高(P<0.01)。与模型组比较,火刃针组和药物组血清及各肌肉组织SOD、T-AOC(药物组肌肉组织除冈下肌外)含量明显增加,TGF-β1、Smad1含量显著降低(P<0.05,P<0.01)。与药物组比较,火刃针组血清和局部肌肉组织SOD、T-AOC(冈上肌除外)、TGF-β1、Smad1含量差异显著(P<0.01)。HE染色结果显示,显微镜下模型组出现肌腱纤维结构明显紊乱,滑膜组织增厚、毛细血管增多、炎性细胞浸润等病理变化。干预后,药物组和火刃针组组织形态学病变显著改善,尤其是火刃针组上述情况明显好转。结论 火刃针可降低肩周炎家兔模型局部组织机化程度,改善病理状态,对血清及局部肌肉组织中的氧自由基代谢发挥良性调节作用,提高抗氧化能力,能够降低TGF-β1/Smad表达水平,对肩周炎家兔模型具有良好的治疗效果。Objective By observing the effects of fireblade needle therapy on shoulder joint histommorphology,antioxidant capacity and transforming growth factor(TGF)-β1/Smad in rabbits with shoulder periarthritis to investigate the intervention mechanism.Methods Male rabbits were divided into blank group,model group,drug group and fireblade needle group,with 8 rabbits in each group according to random number table method.A rabbit scapulohumeral periarthritis model was established by means of continuous traction mechanical exercise strain combined with ice stimulation.The drug group and fireblade needle group were treated with drug intragastric administration and fireblade needle,respectively.Experimental samples were taken on the 33th day of the experiment,and the histopathological changes of biceps tendon and synovial membrane were observed by light microscope after hematoxylin-eosin(HE)staining.The changes of superoxide dismutase(SOD),total antioxidant capacity(T-AOC)and TGF-β1/Smad were detected in serum and biceps,supraspinatus,infraspinatus and teres major muscles.Results Compared with blank group,SOD and T-AOC contents in serum and muscle tissues of model group were significantly decreased,TGF-β1 and Smad1 contents were significantly increased(P<0.01).Compared with model group,SOD and T-AOC contents in serum and muscle tissues of fireblade needle group and drug group(subpost muscle was excluded in the muscle tissue of the drug group)were significantly increased,while TGF-β1 and Smad1 contents were significantly decreased(P<0.05,P<0.01).Compared with drug group,the contents of SOD,T-AOC,TGF-β1 and Smad1 in serum and local muscle tissues in fireblade needle group(except the superior sentry muscle)were statistically significant(P<0.01).The results of HE staining showed that,under the microscope,the model group showed obvious disorder of tendon fiber structure,synovial tissue thickening,increased capillaries,inflammatory cell infiltration and other pathological changes.After intervention,the histomorphological lesion
