艾司氯胺酮对髋关节置换术后轻度抑郁老年患者情绪、睡眠及镇痛的影响

摘　　要：目的分析轻度抑郁老年髋关节置换术患者联合应用艾司氯胺酮的效果,并观察其对睡眠和疼痛程度的影响。方法将我院89例拟行髋关节置换术的老年轻度抑郁患者采用随机数字表法分组,对照组44例予丙泊酚联合围术期心理干预治疗,观察组45例在对照组基础上联合艾司氯胺酮,对比两组患者围手术期心理状态、睡眠质量以及术后短期疼痛程度。结果治疗后,观察组患者焦虑自评量表、抑郁自评量表量表评分较对照组患者低(P<0.05);术后当晚、术后1d及2d观察组患者睡眠质量评分均较对照组患者水平高(P<0.05);术后3h、术后24h及术后48h观察组患者疼痛程度评分均较对照组患者水平低(P<0.05)。结论轻度抑郁老年髋关节置换术患者联合应用艾司氯胺酮具有较好的抗抑郁作用,能有效调节患者的心理状态,减轻术后短期疼痛程度,改善睡眠质量。Objective To analyze the effect and mechanism of esketamine combined with antidepression in elderly patients undergoing hip replacement with mild depression,and to observe its effect on sleep quality and pain degree.Methods 89 elderly patients with mild depression who were about to undergo hip replacement were divided into two groups by random number table method,44 patients in control group were given propofol,45 patients in ob-servation group were combined with esketamine.The mental state,sleep quality and short-term postoperative pain of the two groups were compared.Results After treatment,the scores of self-rating Anxiety scale(SAS)and self-rating Depression Scale(SDS)in observation group were lower than those in control group(P<0.05).Sleep quality scores in observation groups were higher than those in control group on the night after surgery,1d after surgery and 2d after surgery(P<0.05).The pain degree score of observation group was lower than that of control group at 3h,24h and 48h after surgery(P<0.05).Conclusion Esketamine has a good anti-depression effect in elderly patients with hip re-placement combined with mild depression,can effectively regulate their psychological state of patients,reduce their postoperative short-term pain,improve their sleep quality.
