摘　　要：目的探讨焦虑对老年股骨粗隆骨折术后早期康复及疼痛的影响。方法本研究选择2018年1月~2022年12月88例老年股骨粗隆骨折患者,术后当天评估焦虑情绪并设为焦虑组以及非焦虑组,88例患者中术后检出焦虑38例。比较两组患者住院、下床以及开始负重时间,比较术后1d、3d、7d疼痛视觉模拟评分(visual analog scales,VAS)、血清前列腺素E2(prostaglandin E2,PGE2)水平。结果焦虑组住院、下床以及开始负重时间均显著高于非焦虑组(P<0.05);焦虑组术后1d、3d、7d VAS评分以及血清PGE2水平均显著高于焦虑组(P<0.05)。结论老年股骨粗隆骨折术后焦虑发生率较高,并且术后焦虑会延长住院、下床等时间,加重术后疼痛症状,提升血清致痛物质水平,影响患者的近期预后。Objective To investigate the effect of anxiety on early postoperative rehabilitation and pain on elderly patients with femoral fractures after surgery.Methods Eighty-eight elderly patients with trochanteric fracture of femur from January 2018 to December 2022 were selected in this study.Anxiety was assessed on the postoperative day and set into an anxious as well as a non-anxious group,with 38 cases of anxiety detected postoperatively in 88 patients.The time of hospitalization,getting out of bed and starting weight bearing was compared between the two groups.The postoperative visual analog scales(VAS)and serum prostaglandin E2(PGE2)levels were compared at 1,3,and 7 d after surgery.Results The time to hospitalization,getting out of bed,and starting weight bearing was significantly higher in the anxiety group than in the non-anxiety group(P<0.05).Postoperative VAS scores and serum PGE2 levels were significantly higher in the anxious group than in the anxious group at 1d,3d,and 7d postoperatively(P<0.05).Conclusion The incidence of postoperative anxiety is high in elderly femoral trochanteric fractures,and postoperative anxiety prolongs the duration of hospitalization and getting out of bed,exacerbates postoperative pain symptoms,elevates serum levels of pain-causing substances,and affects the patients'immediate prognosis.
