摘　　要：目的探讨脑卒中后偏瘫患者居家康复护理需求的影响因素及各维度关联性。方法选取2022年2月—2023年5月江苏省苏北人民医院收治的脑卒中后偏瘫患者为调查对象,采用一般资料调查量表、《社区脑卒中患者护理需求调查问卷》调查患者的一般资料及居家康复护理需求。采用多因素Logistic回归分析探讨脑卒中后偏瘫患者居家康复护理需求的影响因素;居家康复护理需求各维度的关联性分析采用Apriori关联规则算法。结果本研究共发放调查问卷120份,回收有效问卷115份,有效回收率为95.8%。115例脑卒中后偏瘫患者居家康复护理需求总分为(107.5±23.5)分,其中生活护理需求维度得分为(12.2±3.1)分、健康教育需求维度得分为(20.1±4.2)分、康复指导需求维度得分为(28.0±4.1)分、心理护理需求维度得分为(9.1±2.1)分、专业护理需求维度得分为(5.1±2.0)分、并发症护理需求维度得分为(6.8±1.8)分、自护能力需求维度得分为(13.2±3.3)分、社会支持需求维度得分为(12.9±2.7)分。以居家康复护理需求总分中位数为界,将患者划分为低需求组(n=62,<106分)、高需求组(n=53,≥106分)。两组年龄、文化程度、个人平均月收入及自理情况比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,年龄、文化程度、个人平均月收入是脑卒中后偏瘫患者居家康复护理需求的影响因素(P<0.05)。Apriori关联规则算法结果显示,以康复指导需求为后项,健康教育需求为前项,两项组合出现39次、支持度为24.1%、置信度为88.1%、提升度为1.956。结论脑卒中后偏瘫患者居家康复护理需求处于中高等水平,年龄、文化程度、个人平均月收入是其影响因素;患者存在不同居家康复护理需求,且各需求之间相互促进。Objective To explore the influencing factors of needs of home rehabilitation nursing in patients with hemiplegia after stroke and relevance of various dimensions.Methods Hemiplegia after stroke patients admitted to Northern Jiangsu People's Hospital of Jiangsu Province from February 2022 to May 2023 were selected as the repondents.The General Information Survey Scale and the Community Stroke Patients Care Needs Questionnaire were used to investigate the general information and needs of home rehabilitation nursing of patients.Multivariate Logistic regression analysis was used to explore the influencing factors of needs of home rehabilitation nursing in patients with hemiplegia after stroke.Apriori association rule algorithm was used to explore the relevance of the various dimensions of needs of home rehabilitation nursing.Results A total of 120 questionnaires were distributed in this study,and 115 valid questionnaires were recovered,effective respondent rate was 95.8%.The total score of needs of home rehabilitation nursing of 115 patients with hemiplegia after stroke was(107.5±23.5),in which the score of life nursing demand dimension was(12.2±3.1),the score of health education demand dimension was(20.1±4.2),the score of rehabilitation guidance demand dimension was(28.0±4.1),and the score of psychological nursing demand dimension was(9.1±2.1),the score of professional nursing demand dimension was(5.1±2.0),the score of complication nursing demand dimension was(6.8±1.8),the score of self-care ability demand dimension was(13.2±3.3)and the score of social support demand dimension was(12.9±2.7).The patients were divided into low demand group(n=62,<106)and high demand group(n=53,≥106)based on the median total score for needs of home rehabilitation nursing.There was statistically significant differences in age,education level,personal average monthly income and self-care between the two groups(P<0.05).Multivariate Logistic regression analysis showed that age,education level,personal average monthly income were
