摘　　要：目的探究重症脑梗死患者血清中微小RNA-206(miR-206)和微小RNA-210(miR-210)的表达变化及临床意义。方法选取2021年6月至2022年6月在黄河三门峡医院收治的重症急性脑梗死(ACI)患者96例为重症ACI组,同期收集治疗的轻型ACI患者90例为轻型ACI组,体检健康的志愿者94例为对照组。采用实时荧光定量PCR(qRT-PCR)法检测各组血清miR-206和miR-210的表达水平;采用Pearson法分析重症ACI患者血清miR-206和miR-210表达的相关性;ROC曲线分析血清中miR-206和miR-210的表达水平对重症ACI的诊断价值;多因素Logistic回归分析重症ACI发生的影响因素。结果与轻型ACI组相比,重症ACI组NIHSS评分、脑梗死体积显著升高(P<0.05)。重症ACI组miR-206的表达水平显著低于轻型ACI组及对照组,轻型ACI组显著低于对照组;重症ACI组miR-210的表达水平显著高于轻型ACI组及对照组,轻型ACI组显著高于对照组(P<0.05)。相关性分析显示,重症ACI患者血清中miR-206和miR-210表达水平呈负相关关系(r=-0.366,P<0.001)。二者联合诊断重症ACI的AUC高于miR-206和miR-210单独诊断的AUC值(Z=21.203,P<0.001;Z=35.634,P<0.001)。Logistic回归分析显示,NIHSS评分、脑梗死体积、miR-206和miR-210表达是重症ACI发生的影响因素(P<0.05)。结论重症ACI患者血清中miR-206表达水平呈异常降低,miR-210呈异常升高,两者是重症ACI发生的影响因素。Objective To investigate the expression and clinical significance of microRNA-206(miR-206)and microRNA-210(miR-210)in serum of patients with severe cerebral infarction.Methods A total of 96patients with severe acute cerebral infarction(ACI)admitted to our hospital from June 2021to June 2022were regarded as the severe ACI group,meantime,90patients with mild ACI treated in our hospital were collected as mild ACI group,94healthy volunteers in our hospital were the control group.The expression levels of miR-206and miR-210in serum of each group were detected by qRT-PCR;Pearson method was applied to analyze the correlation between serum miR-206and miR-210expression in patients with severe ACI;ROC curve was applied to analyze the diagnostic value of miR-206and miR-210 expression levels in serum for severe ACI;multivariate Logistic regression was applied to analyze the influencing factors of severe ACI.Results Compared with mild ACI group,the NIHSS score and cerebral infarction volume in severe ACI group were obviously higher(P<0.05).The expression level of miR-206in severe ACI group was obviously lower than that in mild ACI group and control group,and that in mild ACI group was obviously lower than that in control group;the expression level of miR-210in severe ACI group was obviously higher than that in mild ACI group and control group,and that in mild ACI group was obviously higher than that in control group(P<0.05).Correlation analysis showed that the expression levels of miR-206and miR-210in serum of patients with severe ACI were negatively correlated(r=-0.366,P<0.001).The AUC value of the combination of the two methods in the diagnosis of severe ACI was higher than that of miR-206and miR-210(Z=21.203,P<0.001;Z=35.634,P<0.001).Logistic regression analysis showed that NIHSS score,cerebral infarction volume,miR-206and miR-210expression were the influencing factors of severe ACI(P<0.05).Conclusion The expression level of miR-206in the serum of patients with severe ACI is abnormally decreased,and miR-210is abnormally inc
