162例危重症儿童营养状况与全血微量元素的相关性研究

摘  要:目的通过分析危重症住院儿童营养状况与全血微量元素的相关性,为危重症住院患儿微量元素的补充提供依据,确定可能从补充微量元素中受益的患儿人群。方法对2022年6月—2023年6月所有入住苏州大学附属儿童医院PICU的患儿进行回顾性研究,对危重症住院患儿进行营养风险筛查及营养状况评估,并分析营养状况与钙、磷、镁、铜、铁、锌的相关性。结果共纳入162例患儿,营养筛查结果示中等营养不良风险31例(19.16%),高营养不良风险131例(80.84%),患儿营养不良检出率为20.37%。根据营养状况评定分为营养不良组33例(20.37%)和营养正常组129例(79.63%)。对营养不良组患儿及营养正常组患儿微量元素进行比较,提示营养不良组患儿低磷血症发生率更高(P=0.037),且具有更低的铁水平(P=0.007)。结论危重症患儿营养风险及营养不良发生率较高,中重度营养不良患儿相较于营养状况正常患儿铁水平更低,且更易发生低磷血症。临床需及时测定营养状况不佳的危重症患儿的血磷,以便及时补充。Objective To investigate the correlation between nutritional status and whole blood micronutrient in critically ill children,provide evidence for micronutrient supplementation in them,and identify population who may benefit from supplement of micronutrient.Methods All children admitted to PICU from June 2022 to June 2023 in Children's Hospital of Soochow University were observed.It's a retrospective study.Nutritional risk screening and nutritional status assessment for critically ill children were conducted and correlations between nutritional status and CA,P,Mg,Cu,Fe,Zn were analyzed.Results A total of 162 children were enrolled.The results of nutritional screening showed that 31 children(19.16%)were at moderate risk of malnutrition and 131 children(80.84%)were at high risk of malnutrition.The prevalence of malnutrition was 20.37%.According to the assessment of nutritional status,129 patients(79.63%)were in the non-malnutrition group and 33 patients(20.37%)in the malnutrition group.A comparison of the micronutrient between two group showed that the malnutrition group had a higher incidence of hypophosphatemia(P=0.037)and a lower iron level(P=0.007).Conclusion The nutritional risk and the incidence of malnutrition are higher in critically ill children.The iron level of moderate and severe malnutrition children is lower than that of non-malnutrition group,and they are more prone to hypophosphatemia.The blood phosphorus of severe children with poor nutritional status should be measured in time for timely supplement.
