牙体牙髓病患者根管治疗后颞下颌关节紊乱病发生情况研究

摘　　要：目的:探讨牙体牙髓病患者根管治疗后颞下颌关节紊乱病(Temporomandibular disorders,TMD)的发生情况.方法:回顾性收集分析2021年2月至2023年2月期间在本院行根管治疗的110例牙体牙髓病患者的临床资料,根据治疗后TMD发生情况的不同,分为发生组(n=35)和未发生组(n=75).分析影响牙体牙髓病患者根管治疗后颞下颌关节紊乱病发生的相关因素.结果:110例患者治疗后有35例发生TMD,发生率为31.82%(35/110).两组女性、第二磨牙、持续张口时间>1 h、偏侧咀嚼及CPSS评分比较,差异有统计学意义(P<0.05);两组患者其他资料对比,差异无统计学意义(P>0.05).经Logistic回归分析检验结果显示,女性、第二磨牙、持续张口时间>1 h、偏侧咀嚼、CPSS评分高与牙体牙髓病患者根管治疗后发生TMD有关(OR>1,P<0.05).结论:牙体牙髓病患者经根管治疗后,女性、第二磨牙、持续张口时间>1 h、偏侧咀嚼、CPSS评分高等因素均可能会增加治疗后TMD风险.Objective:To investigate the occurrence of Temporomandibular disorders(TMD)in endodontic patients after root canal therapy.Methods:The clinical data of 110 patients with endodontic disease who underwent root canal treatment in our hospital from February 2021 to February 2023 were retrospectively collected and analyzed.According to the incidence of TMD after treatment,they were divided into occurrence group(n=35)and non-occurrence group(n=75).To analyze the related factors affecting temporomandibular joint disorders after root canal treatment in endodontic patients.Results:TMD occurred in 35 of 110 patients after treatment,the incidence was 31.82%(35/110).There were significant differences in CPSS score,second molar,continuous mouth opening time>1 h,hemimastication ratio and CPSS score between the two groups(P<0.05).There was no significant difference in other data between the two groups(P>0.05).Logistic regression analysis showed that TMD after root canal treatment was associated with female,second molar,continuous mouth opening time>1 h,hemimastication and high CPSS score(OR>1,P<0.05).Conclusion:After endodontic root canal treatment,female,second molar,mouth opening time>1 h,hemimastication and high CPSS score may increase the risk of TMD.
