常规135°切口与双陡峭轴透明角膜切口治疗白内障合并中低度数角膜散光的效果比较

摘　　要：目的 比较分析常规135°切口与双陡峭轴透明角膜切口治疗白内障合并中低度数角膜散光的临床效果。方法 选取山东玲珑英诚医院有限公司2020年7月—2023年7月收治的98例白内障合并中低度数角膜散光患者作为研究对象,按照手术方式的不同分为研究组和对照组各49例。研究组实施双陡峭轴透明角膜切口治疗,对照组实施常规135°切口治疗。比较两组术后1周、1个月、3个月的角膜散光和术源性散光情况,术前和术后3个月矫正视力、裸眼视力、主观视觉质量、生活质量情况,以及术后并发症发生情况。结果 术后1周两组角膜散光和术源性散光比较,差异无统计学意义(P>0.05)。术后1个月和3个月,两组角膜散光和术源性散光均较术后1周降低,且研究组低于对照组,差异有统计学意义(P<0.05)。术后3个月,两组矫正视力、裸眼视力、主观视觉质量、生活质量均较术前得到改善,且研究组各项指标均优于对照组,差异有统计学意义(P<0.05)。两组术后并发症发生率差异无统计学意义(P>0.05)。结论 双陡峭轴透明角膜切口治疗白内障合并中低度数角膜散光,较常规135°切口,在改善患者视力和散光方面的临床效果更为理想,可有效提高患者的生活质量和主观视觉质量,且具有较高的安全性,可在临床推广使用。Objective Compare and analyze the clinical effects of conventional 135° incision and double steep axis transparent corneal incision in the treatment of cataract with moderate to low degree corneal astigmatism.Methods A total of 98 cataracts patients with moderate to low degree corneal astigmatism admitted to Shandong Linglong Yingcheng Hospital from July 2020 to July 2023 were selected as the research subjects.They were divided into a study group and a control group,with 49 cases in each group,according to different surgical methods.The study group underwent double steep axis transparent corneal incision treatment,while the control group underwent conventional 135° incision treatment.Compare the corneal astigmatism and surgical induced astigmatism between two groups at 1 week,1 month,and 3 months after surgery.Compare the corrected visual acuity,naked eye vision,subjective visual quality,and quality of life between two groups before and 3 months after surgery.Compare the incidence of postoperative complications between two groups.Results There was no significant difference in corneal astigmatism and surgical induced astigmatism between the two groups at 1 week after surgery(P>0.05).At 1 and 3 months after surgery,corneal astigmatism and surgical induced astigmatism in both groups decreased compared to 1 week after surgery,and the study group was significantly lower than the control group(P<0.05).After 3 months of surgery,the corrected visual acuity,naked eye vision,subjective visual quality,and quality of life in both groups improved compared to before surgery,and all indicators in the study group were significantly better than those in the control group(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion The double steep axis transparent corneal incision treatment for cataracts with moderate to low degree corneal astigmatism has a more ideal clinical effect than the conventional 135° incision in improving patient vision and asti
