摘　　要：目的根据双向两样本孟德尔随机化(Mendelian randomization,MR)方法,探讨口腔溃疡与乳腺癌之间的因果关系。方法利用大规模遗传数据集,进行双向MR分析,使用暴露数据集和结果数据集推断因果关系。确定与口腔溃疡相关的遗传工具变量,并检查其与乳腺癌的关联。为确保因果关系推断的稳健性,使用孟德尔随机化Egger(MR egger)法、逆方差加权法(inverse variance weighted,IVW)、加权中位数法(weighted median,WM)、简单模式和加权模式等多种MR方法。此外,进行异质性和多向性检验,来评估MR结果的可靠性。结果MR显示,当口腔溃疡被视为暴露因素时,口腔溃疡与乳腺癌之间存在负相关(OR 0.396;95%CI 0.175至0.897;P<0.05)。在逆向MR中未观察到支持乳腺癌对口腔溃疡产生影响的显著结果(所有P>0.05)。结论通过MR方法证明口腔溃疡可能是乳腺癌的保护因素,在探究口腔溃疡与乳腺癌关系时需要综合多种研究方法。Objective Utilizing a two-way two-sample Mendelian randomization(MR)approach,this study aims to investigate the causal relationship between oral ulcers and breast cancer.Methods Employing data from a largescale genetic dataset,a bidirectional MR analysis was conducted using an exposure dataset and an outcome dataset to infer causal relationships.Genetic instruments associated with oral ulcers were identified,and their association with breast cancer was examined.To ensure the robustness of causal inference,multiple MR methods,including MR egger,inverse variance weighting(IVW),weighted median(WM),simple mode,and weighted mode,were employed.Additionally,heterogeneity and multidirectionality tests were performed to assess the reliability of MR results.Results The MR analysis revealed a suggestive negative association between oral ulcers and breast cancer when oral ulcers were considered as an exposure factor(OR 0.396;95%CI 0.175 to 0.897;P<0.05).No significant results supporting the effect of breast cancer on oral ulcers were observed in the inverse MR(all P>0.05).Conclusion Using MR methods,it was demonstrated that oral ulcers may serve as a protective factor against breast cancer,emphasizing the importance of integrating various methodologies in exploring the relationship between oral ulcers and breast cancer.
