《针灸大成》中大陵穴的临床应用规律探析

摘　　要：目的:《针灸大成》是一部中国古代针灸经典,其中对于大陵穴的应用有详细的论述。文章旨在整理并分析《针灸大成》一书中关于大陵穴的记载,并归纳总结其穴名、定位、归经、刺灸方法及配伍规律,为临床应用大陵穴提供参考和理论依据,以推广大陵穴及其针灸治疗的方法。方法:以《针灸大成》为文献检索来源,检索词为“大陵”,采用文献资料分析法,通过计算机检索及人工检索相结合的方式筛选、分析《针灸大成》中关于大陵穴的条文,建立相应的数据库,并应用IBM SPSS Modeler 18.0及Cytoscape_v3.9.1数据处理软件进行数据处理、频次分析,归纳总结大陵穴的穴名、定位、治法、配伍规律及适应病症等,并结合现代医学相关研究分析大陵穴的作用机制。结果:大陵穴位于腕掌侧远端横纹中,掌长肌腱与腕屈肌腱之间,可针可灸可推。常规针刺深度为针针三至五分,常规灸量为三壮。大陵穴长于治疗心系病症,对于疼痛类疾病、消化系统疾病、外科疮疡等亦有较好的疗效。结论:大陵穴首见于《黄帝内经》,是临床常用穴位且是十三鬼穴之一,在临床应用中具有广泛的适应证和显著疗效。临床应用大陵穴时应根据具体病症施治,配合其他穴位的应用,以取得更好的治疗效果。Objective:Zhenjiu Dacheng(《针灸大成》) is a classic of acupuncture and moxibustion in ancient in China,in which the application of the Daling points(PC7) is discussed in detail.The purpose of this paper is to sort out and analyze the records about Daling points in Zhenjiu Dacheng,and summarize its name,location,meridian orientation,acupuncture and moxibustion methods and compatibility rules,so as to provide reference and theoretical basis for clinical application of the Daling points,and promote the Daling points and its acupuncture and moxibustion treatment.Methods:Taking Zhenjiu Dacheng as the source of document retrieval,with the key word of Daling,the item about the Daling points in Zhenjiu Dacheng was screened and analyzed by means of computer search and manual search,and the corresponding database was established by IBM SPSS Modeler 18.0 and Cytoscape_V3.9.1 Data processing software to conduct data processing and frequency analysis,summarize the acupoint name,location,treatment,compatibility rules,and indications of Daling acupoints,and analyze the mechanism of action of the Daling acupoint in combination with modern medical research.Results:The Daling acupoints are located in the transverse line at the distal end of the palmar side of the wrist,between the palmaris longus tendon and the flexor carpi tendon,and the methods acupuncture,moxibustion,or pushcan be performed on then.The conventional acupuncture depth is about 0.99 ~ 1.67 cm equal to of the needle,and the conventional moxibustion amount is 3 times.The Daling acupoint is good for treating heart diseases and has good therapeutic effects on pain related diseases,digestive system diseases,surgical ulcers,etc..Conclusion:The Daling points,first seen in Huangdi Neijing(《黄帝内经》),is one of the thirteen ghost points commonly used in clinical practice and has extensive indications and significant curative effects in clinical application.When applying the Daling acupoint in clinical practice,treatment should be based on specific symptoms a
