摘 要:目的探讨抑郁症(MDD)患者脑灰质体积变化与睡眠质量、认知功能的关系。方法选取50例MDD患者作为研究组,同时随机选取50名健康人员作为对照组,使用GE Signa Pioneer MRI仪获取研究对象脑影像数据经SIENAX软件分析观察两组研究对象脑灰质体积差异;记录两组研究对象的睡眠质量和认知功能。结果研究组左侧前扣带回、左侧颞中回、双侧前额叶、左侧丘脑的脑灰质体积均低于对照组(P<0.05)。研究组PSQI各因子评分均高于对照组(P<0.01)。研究组MoCA各因子评分均低于对照组(P<0.01)。左侧颞中回与PSQI评分中日间功能评分无相关性(P>0.05),而与其他维度评分呈负相关性(P<0.05);左侧前扣带回、双侧前额叶、左侧丘脑与PSQI评分均呈负相关性(P<0.05)。双侧前额叶与MoCA评分中命名、注意评分无相关性(P>0.05),而与其他维度评分呈正相关性(P<0.05),左侧前扣带回、左侧颞中回、左侧丘脑与MoCA评分均呈正相关性(P<0.05)。结论MDD患者存在多个与认知、睡眠相关的脑灰质体积减低情况,与认知功能呈负相关,与睡眠质量呈正相关,脑灰质体积变化对MDD患者有一定的诊治预测价值,能够客观地反应认知功能改变及睡眠障碍情况。Objective To explore the relationships among changes in gray matter volume,sleep quality and cognitive function in patients with depression(MDD).Methods A total of 50 MDD patients in study group,and 50 healthy controls randomly selected in control group were selected.Their brain imaging data were obtained through GE Signa Pioneer MRI,and the differences in gray matter volumes between the two groups were analyzed and observed using SIENAX software.The sleep qualities and cognitive functions of two groups were recorded.Results The gray matter volumes of the left anterior cingulate gyrus,left middle temporal gyrus,bilateral prefrontal lobes and left thalamus in study group were all lower than those in control group(P<0.05).All the factor scores of PSQI in study group were all higher than those in control group(P<0.01).All the factor score of MoCA in study group were all lower than those in control group(P<0.01).The left middle temporal ghyrus was negatively correlated with all the factor scores of PSQI(P<0.05),except the factor score of daytime function(P>0.05).The left anterior cingulate gyrus,bilateral prefrontal lobes and left thalamus were all negatively correlated with all the factor scores of PSQI(P<0.05).The bilateral prefrontal lobes was positively correlated with all the factor scores of MoCA(P<0.05),except the factor scores of naming and attention(P>0.05).The left anterior cingulate gyrus,left middle temporal gyrus and left thalamus were all positively correlated with all the factor scores of MoCA(P<0.05).Conclusion MDD patients have multiple decreases in gray matter volume related to cognition and sleep,which is negatively correlated with cognitive function and positively correlated with sleep quality.Changes in gray matter volume have certain diagnostic and predictive value for MDD patients,and can objectively reflect changes in cognitive function and sleep disorders.
