电针联合A型肉毒毒素肌肉注射治疗痉挛型脑瘫患儿的临床效果及对粗大运动功能的影响

摘　　要：目的观察电针联合A型肉毒毒素肌肉注射治疗痉挛型脑瘫患儿的临床效果及对粗大运动功能的影响。方法选取2021年7月至2023年11月本院收治的96例痉挛型脑瘫患儿为研究对象,按照随机数字表法将其分为对照组和观察组,每组48例。对照组采用A型肉毒毒素肌肉注射配合常规康复训练,观察组在对照组的基础上予以电针治疗。比较两组的治疗效果。结果治疗后,两组的改良Ashworth痉挛量表(MAS)评分、足背屈角、88项版本粗大运动功能测试量表(GMFM-88)评分均优于治疗前(P<0.05);治疗后,两组的MAS评分、足背屈角无显著差异(P>0.05),但观察组的GMFM-88评分明显高于对照组(P<0.05)。结论电针联合A型肉毒毒素肌肉注射治疗痉挛型脑瘫患儿效果显著,可以快速降低肢体肌张力,改善尖足,提高粗大运动功能,值得临床推广及应用。Objective To observe the clinical effect of electroacupuncture combined with intramuscular injection of botulinum toxin A in the treatment of children with spastic cerebral palsy and its influence on gross motor function.Methods Ninety-six children with spastic cerebral palsy admitted in our hospital from July 2021 to November 2023 were selected as the study objects and divided into control group and observation group according to random number table method,with 48 cases in each group.The control group was treated with intramuscular injection of botulinum toxin A and routine rehabilitation training,and the observation group was treated with electroacupuncture on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the Modified Ashworth Spasticity Scale(MAS)score,dorsiflexion angle of foot and 88 Items Version of Gross Motor Function Measure(GMFM-88)score in both groups were better than those before treatment(P<0.05);after treatment,there were no significant differences in the MAS score and dorsiflexion angle of foot between the two groups(P>0.05),but the GMFM-88 score in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Electroacupuncture combined with intramuscular injection of botulinum toxin A is effective in the treatment of children with spastic cerebral palsy,it can rapidly reduce limb muscle tension,improve tiptoe and gross motor function,which is worthy of clinical promotion and application.
