摘 要:目的:分析血清胱抑素C(Cys-C)水平与肾癌患者预后的关系.方法:回顾性分析2018年1月—2019年6月于麻城市人民医院接受治疗的78例肾癌患者的临床资料,根据患者术前检查中血清Cys-C水平将入选患者分为正常组(49例)与高表达组(29例);随访3年,统计患者预后,根据末次随访结果将全部患者划分为预后良好与预后不良,比较预后良好及预后不良患者Cys-C水平,以COX模型分析血清Cys-C水平与患者预后的关系.结果:高表达组年龄大于正常组,病理分期、开放术式占比及尿素、肌酐、尿酸水平均高于正常组,肾小球滤过率、预后良好率低于正常组,差异均有统计学意义(P<0.05);预后良好患者血清Cys-C水平低于预后不良患者,差异有统计学意义(P<0.001);COX模型分析结果显示,Cys-C高表达为肾癌患者不良预后的独立危险因素(OR>1,P<0.001).结论:血清Cys-C水平呈异常高表达为肾癌不良预后的独立危险因素,可导致肾癌患者肾功能损伤风险升高.Objective:To analyze the relationship between serum cystatin C(Cys-C)level and prognosis of patients with renal cancer.Methods:The clinical data of seventy-eight patients with renal cancer who received treatment in Macheng People's Hospital from January 2018 to June 2019 were retrospectively analyzed.According to the serum Cys-C level of the patients in the preoperative examination,the enrolled patients were divided into normal group(49 cases)and high expression group(29 cases).After 3 years of follow-up,all patients were classified into good prognosis and bad prognosis according to the results of the last follow-up.Cys-C levels were compared between patients with good prognosis and patients with poor prognosis.COX model was used to analyze the relationship between serum Cys-C levels and prognosis.Results:The age of the high expression group was longer than that of the normal group,the pathological stage,the proportion of open surgery and the levels of urea,creatinine and uric acid were higher than those of the normal group,and the glomerular filtration rate and good prognosis rate were lower than those of the normal group,with statistical significance(P<0.05);Serum Cys-C level in patients with good prognosis was lower than that in patients with poor prognosis,and the difference was statistically significant(P<0.001);COX model analysis showed that high expression of Cys-C was an independent risk factor for poor prognosis in patients with renal cancer(OR>1,P<0.001).Conclusion:Abnormally high expression of serum Cys-C is an independent risk factor for poor prognosis of renal cancer and may lead to increased risk of renal impairment in patients with renal cancer.
